Clinical Observation On Prevention Of The Adhesions After TKA With Medical Sodium Hyaluronate Gel

BackgroundNow total joint arthroplasty has been the main method to correct joint deformity, relieve joint pain and reconstruct joint function.The knee joint was request to have the fuction of weight-bearing,extention,flexion,abduction,rotation and stability after total knee arthroplasty(TKA).The early rehabilitation activities can improve the knee fuction obviously after TKA and has been the main factor which affect the operation effectiveness and the long-term fuction of the knee.The joint adhesions of the surrounding soft tissue was one of the important factors which affect the early rehabilitation activities.PurposeTo analyze the contribution and possible mechanism of the using of Medical sodium hyaluronate gel in TKA to prevent joint adhesions by observation and comparison the early result.Methods1.A total of 86 patients undergoing unilateral TKA for idiopathic OA at Henan Province Hospital orthopedics department were surveyed from June 2010 to February 2011.There are 30 male and 56 female which had a follow up from 1 to 3 month.The age is from 58 y to 72 y and average age is 65.4 y.2.Grouping:First separate into two groups in order to eliminate gender affect.Then separate into control groups and treatment groups together 4 groups.2.1 Male control group(m1):which didn't use the HA to prevent adhesions after TKA. 3 2.2 Male treatment group(m2):which use the HA to prevent adhesions after TKA.2.3Female control group(wl):the same as m1.2.4Female treatment group(w2):the same as m2.3. Evaluate the knee joint function preoperative and two weeks afteroperation accord to the Hospital for special surgery knee score(HSS) and American knee society knee score(KSS). Then find the significative indications to analyze the statistical significance.Results1.All patients'postoperative HSS score were higher than preoperative. There were statistical differences among them (P<0.01). All patients'postoperative KSS score no matter clinical score or function score were higher than preoperative. There were also statistical differences among them (P<0.01).2.The postoperative HSS activity score and postoperative KSS activity score of m2 group were both higher than ml group. There were statistical differences among them (P<0.05). The postoperative HSS activity score and postoperative KSS activity score of w2 group were also both higher than w1 group. There were also statistical differences among them (P<0.05).The result was in conformity with clinical observation.Conclusions1.It was a simple,secure and effective therapeutic method to prevent joint adhesions after TKA using the Medical sodium hyaluronate gel.It can promote the patient's recovery and improve the quality of life postoperative.2.The using of Medical sodium hyaluronate gel after TKA can improve the activity degrees of the knee joint.This should be helpful to the knee function after TKA.3.TKA should be the first choice of serious OS patients for improving the knee function and knee score evidently.
