Objective:To observe the guidance on the function recovery and the change of electrophysiology of low back muscle after operation of the middle protrusion of the lumbar disc herniation.Methods:Chose 70 cases waist 4/5 disc herniation postoperative patients that were accord with the diagnostic criteria and the inclusion criteria and from February 2009 to February 2010. Divided into guidance group (group A) and the control group (group B). Giving group A guidance instruction, Giving Group B routine rehabilitation therapy, no other interention. Used surface electromyography apparatus to detect Group A and Group B'MFs and FER on 7 days,4 weeks,6 months after surgery respectively.Used EMG/muscle electric inducing apparatus to inspect the degree and the number of spontaneous action potentials of the erector spinae of both groups. Evaluated clinical symptom scores with JOA criteria to Group A and Group B. The above data were statistically analyzed. Discussed the mechanism of guidance promoted low back muscle functional recovery finally.Results:(1) There is no significant difference of the degree and the number of spontaneous action potentials of the erector spinae,JOA criteria in GroupA and GroupB on Day 7 after surgery,(p>0.05).(2) There is significant difference of FER,MFs,the degree and the number of spontaneous action potentials of the erector spinae,JOA criteria in GroupA and GroupB on 4 weeks after surgery, (p<0.05).(3) There is highly significant difference of FER,MFs,the degree and the number of spontaneous action potentials of the erector spinae,JOA criteria in GroupA and GroupB on 6 months after surgery, (p<0.01). (4) There is significant difference of JOA criteria in GroupA between on Day 7 and 4 weeks, (p<0.05); There is highly significant difference of JOA criteria in GroupA between on 4 weeks and 6 months, (p<0.01); There is significant difference of JOA criteria in GroupB between on Day 7 and 4 weeks, (p<0.05); There is highly significant difference of JOA criteria in GroupB between on 3 months and 6 months, (p<0.01).(5) There is no significant difference of JOA criteria between GroupA and GroupB on Day 7 after surgery,(p>0.05); There is significant difference of JOA criteria between GroupA and GroupB on 4 weeks after surgery, (p<0.05); There is highly significant difference of JOA criteria between GroupA and GroupB on 6 months after surgery, (p<0.01).Conclusions:The guidance showed a significant improving effect on the function of the low back muscle restored after operation of the middle protrusion of the lumbar disc herniation.
