BackgroundVenous thromboembolism (VTE), including deep vein thrombosis(DVT) and pulmonary embolism(PE), is a frequent complication in cancer patients and is one of the main factos which lead to the death of cancer patients (the second leading cause of death in patients with cancer). About 4%-20% cancer patients can occur VTE and of all patients presenting with acute VTE,20% are associated clinically with malignancy;2%-5% of these are diagnosed concurrently with cancer, and 5%-10% are diagnosed with cancer during follow-up. Cancer patients are at a higher risk of VTE than patients without cancer. It represents an important cause of morbidity and mortality. They interact and influence each other as well. Large databases Analysis has indicated that cancer patients who either develop a VTE during the course of established cancer or are diagnosed with a thrombotic episode when cancer is first diagnosed have poorer survival compared with cancer patients without thrombosis.A substantial body of literature has developed showing that malignancy is not only a hypercoagulable state characterized by an increased risk of thrombosis but also that components of blood coagulation reactions are capable of supporting tumor growth and dissemination. So the prevention of VTE is an important component of the clinical treatment of cancer. No clear agreement for the prevention and control of the Tumor thrombosis, especially in preventing thrombosis and improving high pour-point state,has been made in international. No enough attention has been payed to the Tumor thrombosis in our country and very few report can be seen about the inceidence of the tumor thrombosis. In order to improve the understanding of the risk of the tumor thrombosis and to explore methods to the prevention and control of the tumor thrombosis, we analysed 9 tumor thrombosis cases in our department between Oct.2009 and Dec.2010.ObjectiveTo learn the risk factors,clinical manifestations, diagnosis and treatment of the tumor thrombosis and also the relationship between the thrombosis and prognosis of tumor.MethodsAnalysing the clinical manifestations, related Blood biochemical instructions, related Imaging examination, diagnosis, treatment and outcomes of 9 tumor thrombosis cases in our department between Oct.2009 and Dec.2010. Discussing the risk factors,clinical manifestations, diagnosis, treatment and the relation to the prognosis of the tumor thrombosis.ResultsMalignancy has a hypercoagulable state which increase the risk of thrombosis especially among patients who were accepting chemotherapy in the hospital. Among the 9 cases,one breast cancer patient with PTE was too serious to receive the anticoagulant and thrombolytic therapy and died for the PTE. Another diffuse large B cell lymphoma(DLBCL) patient with DVT of left lower limb received the therapy of Low molecular heparin calcium for two days and refused to the continued therapy for economic reasons. Then the swelling of the left lower limb became worse and the color doppler ultrasound proved the Thrombus block be worse than before. The remaining 7cancer patients with DVT received the therapy of Low molecular heparin calciumand urokinasebecame stable and the swelling and pain of the limb reduced or disappeared.Color doppler ultrasound showed the thrombosis became smaller than before or disappeared.ConclusionsThrough the analysis and summary of tumor thrombosis cases, we learned the risk factors,clinical manifestations, diagnosis, treatment of the tumor thrombosis and also the relationship between the thrombosis and prognosis of tumor,which have a great significance to raising the awareness of prevention of tumor thrombosis, improving the treatment level of tumor thrombosis, reducing morbidity and mortality of tumor and improving tumor prognosis.
