Objective Through the change of coagulation function,platelet and quality of Life rating before and after the treatment,observe the effect of acupuncture on hypercoagulability of blood and the curative effect compared with the conventional western medicine treatment methods.Methods Ninety-six patients with malignant tumor in hypercoagulable state were randomly divided into western medicine group and acupuncture medicine group with 48 cases each.Both groups were on the basis of conventional symptomatic treatment.Western medicine group was treated with low-molecular-weight heparin only once a day for 3 months.Acupuncture medicine group was treated with low-molecular-weight heparin combined with acupuncture at Qihai and Yushu(bilateral),Zusanli(bilateral),Sanyinjiao(bilateral),blood sea(bilateral),each treatment time is 30 minutes,once every other day for 3 months.Through the changes of coagulation function,platelet and KPS scores to observe the effect of acupuncture on the hypercoagulable state of malignant tumors and the comparison with the conventional western medicine treatment methods.Results 1.Baseline comparion:(1)The two groups was no statistical differences in age,height,weight,sex,surgery,tumor metastasis,clinical stage and catheterization,the distribution of primary tumor site,clinical stage and syndrome differentiation of traditional Chinese medicine when they entered the groups(P>0.05),results comparable.(2)There was no statistical difference in the laboratory parameters(APTT,PT,TT,DD,FIB,PLT)between the two groups when they entered the groups(P>0.05),results comparable.(3)There was no statistical difference between the two groups in KPS score and quality of life measurement when they entered the groups(P>0.05),which was comparable.2.The main efficacy indicators:(1)Comparison of PT between two groups: 1)There was no significant difference between the two groups in the change of PT between groups and interaction effect(P>0.05),but there was statistical difference in different treatment time.(P<0.05).2)Acupuncture medicine group after 3 months of treatment have statistically significant differences compared with before treatment(P <0.05).(2)Comparison of APTT between the two groups: 1)There was no significant difference in the interaction effect between the two groups(P>0.05),but there were statistical difference between the two groups and time(P<0.05).2)There were statistical differences between the two groups after treatment for 3 months compared with before treatment(P<0.05).There was a statistically significant difference between the two groups after 3 months of treatment(P<0.05).(3)Comparison of DD between two groups: 1)There was no significant difference(P>0.05)in the change of the DD between the two groups at the time,between the two groups and the interaction effect.2)There were statistically significant differences between the two groups after 1,2 and 3 months of treatment compared with before treatment(P<0.05).There was a statistically significant difference between the two groups after 3 months of treatment(P<0.05).3.Secondary efficacy indicators:(1)There was no significant difference in the changes of FIB,TT and PLT between the two groups(P>0.05),but there was significant difference in different treatment time(P<0.05).(2)FIB: There were statistically significant in the western medicine group compared with before treatment after 2 and 3 months of treatment(P <0.05),acupuncture medicine group after treatment in 3 months have statistical differences compared with before treatment(P<0.05).TT: There was no statistical difference between the two groups in the TT within the group and the same treatment time point(P> 0.05).PLT: After 2 and 3 months of treatment,there was significant difference between the two groups compared with before treatment(P<0.05).4.Changes in the scale:(1)Comparison of KPS between the two groups: There was no statistically significant difference before and after treatment(P > 0.05).(2)Comparison of two groups of patients’ quality of life measurement scales(EORTC QLQ-C30): Repeated measures analysis of variance showed: 1)The range of changes in somatic function of the two groups have statistical differences in time,group,and interaction effects(P<0.05).There was no significant difference in the change of social function between the two groups in time,group,and interaction(P<0.05).There was no significant difference in the changes in roles,emotions,cognitive functions,and overall life between the two groups and interaction effects(P>0.05),but there was a statistically significant difference in the different treatment time(P<0.05).2)After 1 month,2 months and 3 months of treatment,there were statistically significant differences between the two groups in terms of body,character,emotion,cognition,social function,and overall life compared with before treatment(P<0.05).The physical function of the two groups was statistically different between the 2 and 3 months after treatment(P<0.05).5.Safety assessment: compare the two groups of patients with hemorrhagic adverse reactions(skin petechia petechiae,bleeding gums,melena,subcutaneous sclerosis).the experimental group haven’t occurred adverse events;control group occurred in 6 cases(13.6%).Conclusion 1.Acupuncture medicine group reduced the hypercoagulable state of malignant tumors D-dimer content was better than western medicine group;2.The western medicine group and the acupuncture medicine group have the same efficacy in improving the remaining indexes of the hypercoagulable state coagulation indexes of malignant tumors;3.The effect of the acupuncture and medicine group on improving physical function in the quality of life is better than that of the western medicine group;4.The Western medicine group and the acupuncture group have equivalent efficacy in improving the remaining indicators in the quality of life.
