Objective:To assess the therapeutic effect of Tolterodine combined bladder trains on Urge Urinary Incontinence .Methods:56 female cases of Urge Urinary Incontinence were divided into three groups as group A (17 cases), B (19 cases), and C (20 cases). For group A, the patients were instructed to follow the prescription of Tolterodine 2mg p.o. bid. For group B, they were required to carry out bladder training in which they drank more water in the daytime, tried to postpone the urination as long as possible up to over 300 ml of the per urine output gradually, and drank no water at nightfall. In this way, they were finally trained to urinate per 3 to 4 hours. The following directions had to be followed during the bladder training. First, the patients start off the training from the morning. Secondly, they evacuate the bladder in the bathroom as soon as they get up. Thirdly, in the daytime, the clear-headed patients go to urinate thoroughly at the required intervals on a regular basis. Fourthly, there is no drill at night. Finally, should there be urge to urinate prior to the appointed time, they divert their attention and try to urinate until the appointed time to the greatest extent. For group C, they were demanded to take Tolterodine 2mg p.o. bid combined with bladder training. Before normal treatment, the selected patients took notes of the daily micturition frequency and urinary incontinence for a week. The total course of treatment lasted eight weeks, in which the patients by their own wrote the urination diary covering the micturition frequency as well as urinary incontinence.Results:After three groups of patients were treated by the three methods, the therapeutic effect on reducing frequency of micturition on the average 24 hours is: group C>group A>group B; in the same group, the therapeutic effect from 5th to 8th week is better than that from 1st to 4th week, the overall therapeutic effect after 8 weeks is much better than before (P<0.05). After three groups of patients were treated by the three methods, the therapeutic effect on reducing frequency of incontinence on the average 24 hours is: from 1st to 4th week, both group A and group C are better than group B(P<0.05),without statistical significance between group A and group C(P>0.05); from 5th to 8th week, group C is better than group A and group B(P<0.05), without statistical significance between group A and group C(P>0.05); after 8-week-course of treatment: group C>group A>group B(P<0.05). The comparison of the therapeutic effect in the same group rests with the following. The therapeutic effect from 5th to 8th week is better than that from 1st to 4th weeks, and frequency of incontinence of the two groups on the average 24 hours is lower than the previous treatment; the therapeutic effect on reducing frequency of micturition on the average 24 hours after 8 weeks is better than before(P<0.05).Conclusion:Taking Tolterodine combined with bladder training is more effective than simply taking Tolterodine or bladder training for the treatment of female urge urinary incontinence.
