| Objective:To investigate the onset characteristics of gastric cancer of young and old patients,to try to improve the recognition of their characteristics and increase the rate of early diagnosis for gastric cancer especially for young gastric cancer patients.Methods and Object:To retrospectively analyze the epidemiology and clinicopathologic characteristics of young and old gastric cancer patients respectively, including the suffers gender, age, family history,, clinic symptom,and false positive rate, pathological changes location, roughly gastroscope type, histology type, clinical staging, surgery and permanent control degree et al.Tumor staging is according to TNM staging standard of UICC. Studying targets are the gastric cancer patients of the second affiliated hospital of Nanchang University which treated during from January 2005 to December 2009 and have the complete pathology inspection material choose the age section separately to makes the data statistical analysis below 35 years old and above 60 years old of patients, and the age≤35 years old gastric cancer patients are defined as the youth group, then the age bigger than 60 years old are defined as the old age group, the data statistical analysis uses the X2 examination, P＜0.05 is considered has the statistical significance.Results:(1) Sex,age and family history: stomach cancer overall disease incidence rate by masculine majority, but among the young people patients female is more than the male, then among the old age group the male is taken the majority; morbidity age section are mostly taken by over 50 years old; in the young age group 11(12.9%)examples have famliy history, by the old age group is total taken 23(3.0%); (2) Clinical symptoms and misdiagnostic rate:the cardinal symptoms of two groups are anemia,the epigastrium ache and discomfort, in the old age group many surferer have the dysphagia symptom; and the total false diagnosis rate of the young age group are higher than the old age group, thereinto "digestive ulcer " misdiagnosed differences has statistics significance;(3) Pathological change spot:the youth group morbidity spot by sinuses ventriculi, the corpora ventriculi primarily, and the old age group by the cardia, the fundus ventriculi and sinuses ventriculi is taken primarily; (4) Rough type by gastroscope:the diagnosis rate of two group of early gastric cancer have no obvious statistics difference, the progressive stomach cancer in the youth group Borrmann III and Borrmann IV stomach cancer is taken primarily, then in the old age group is Borrmann II and Borrmann III,but Borrmann I rate is more higher than young age group; (5)Histology type:the young age people stomach cancer is taken the lower differentiated adenocarcinoma and signet ring cell cancer primarily, but the old age group by middle differentiated adenocarcinoma primarily; (6) Clinical staging:on the clinical stagings hand,two groups of morbidity are all by clinical stagesⅢ,stagesIV mostly, but in the youth group, stage IV morbidity are obviously higher than the old age group; (7) Surgical operation and radical cure degree comparing:the surgical operation rates of two group of surferer have no significant difference,but the youth group's radical cure rate is more obviously lower than an old age group, and the palliative operation rate is more obviously higher than old age group, and the differences have statistics significance.Conclusion:The stomach cancer overall morbidity are mostly taken by the middle-aged person and old person, the youth as well as the senior citizen stomach cancer morbidity exist different sex distributed difference possibility, the youth patient has bigger possibility of have the stomach cancer family history;the stomach cancer do not have the specificity clinical manifestation, the old age patient top digit cancer disease incidence rate are high;compared with old age patient,youth stomach cancer patient prognosis is relatively worse. |
