Objective 1. Analyze the clinical characteristics of gastric cancer patients of different ages. 2. Analyze the operation risk of gastric cancer patients.Methods 1. The data of 463 gastric cancer patients underwent gastrectomy were analyzed retrospectively. The patients were divided into 3 groups according to age: ≤65 years old group, 65-80 years old group and ≥80 years old group. The clinical and pathological data and the clinical characteristics were analyzed. 2. The data of 463 gastric cancer patients underwent gastrectomy were analyzed retrospectively. The patients were divided into 2 groups according to postoperative complication level: severe postoperative complication group and control group. The clinical and pathological data and the clinical characteristics were analyzed. The operation risk was predicted.Results 1. There was no significant difference in the constituent ratio of male and female, operative method, operative time, blood loss, pathological characteristics, number of postoperative complications, postoperative Clavien-Dindo grade and postoperative hospital stay between patients of different ages(p>0.05). Patients in ≥80 years old group have more preoperative complications than those in the other two groups(p<0.05). 2. There was no significant difference in the age, sex, tumor invasion, AJCC TNM stage, extent of lymph node dissection, operative time, hypoproteinemia and diabetes mellitus between patients of severe postoperative complication group and control group(p>0.05). There was significant difference in the lymphatic metastasis, combined resection, total gastrectomy, heart disease, lung disease, hypertension and anemia between patients of the two groups(p<0.05). Univariable analysis showed that severe postoperative complication is associated with lymphatic metastasis, combined resection, total gastrectomy, heart disease, lung disease, hypertension and anemia(p<0.05); multivariable analysis showed that severe postoperative complication is associated with combined resection, total gastrectomy, lung disease, hypertension and anemia(p<0.05). The formula(Gastric Cancer Operation Risk Score = lung disease score + hypertension score + anemia score + total gastrectomy score + combined resection score) has high sensitivity, specificity and Youden’s index.Conclusion 1. Older gastric cancer patients have more preoperative complications, but the number and severity of postoperative complications are not increased. 2. Severe postoperative complications are associated with combined resection, total gastrectomy, lung disease, hypertension and anemia. Severe postoperative complications can be predicted simply and precisely by the Gastric Cancer Operation Risk Score.
