Analysis Of The Relationship Between Clinical Pathological Characteristics And HER-2 Expression And Prognosis Of Gastric Signet-ring Cell Carcinoma

Objective:The objective of this study was to analyze the correlation between HER-2 expression and clinicopathological parameters in gastric signet-ring carcinoma tissues and its effect on prognosis.Methods:We retrospectively the msdical records of 274 patients diagnosed gastric signet-ring cancer who fulfilled the specified criteria was performed from January 2011 to December 2012 in Wannan Medical College First Affiliated Yijishan hospital,and used immunohistochemistry(IHC)to detect the expression of HER-2 protein.We tested the HER-2 gene magnification of the immunohistochemical suspicious positive specimens by fluorescence in situ hybridization(FISH).The study analysis the correlation of HER-2 expression and clinicopathological parameter in gastric signet-ring carcinoma tissue and its effect on prognosis.Result:By using immunohistochemistry(IHC)and fluorescence in situ hybridization(FISH)to test the expression of HER-2,the positive rate was 14.6%(40/274).The positive rate of HER-2 in classical type(type ?)SRC was 20.4%(11/54),in tissue cell type(type ?)and cytoplasmic eosinophilic type(type ?)SRC was 21.7%,in small cell type(type ?)and inter cell type(type ?)SRC was 9.3%(14/151),it had sense among them(P=0.021).Comparison of two,the positive rate of type ? and ? SRC was higher than the positive rate of type ? and ? SRC,the difference was statistically singnificant(P=0.011).But it had no sense between type ? and type ? and ?(P=0.854);There was no significant difference in the positive rates of type ? and type ? and ? gastric signet-ring cell carcinoma(P=0.854,P=0.032).The positive rate of male was 15.4%(31/201),the positive rate of female was 12.3%(9/73),the difference was not singnificant(P=0.521).The positive rate of less than sixty years old was 14.9%(13/87),the positive rate of over sixty years old was 14.4%(27/187),the difference was not singnificant(P=0.912).The positive rate of cardia and fundus ventriculi was 15.3%(13/85),the positive rate of corpora ventriculi and stomach angle was 10.7%(8/75),the positive rate of pylorus and sinuses ventriculi was 9.6%(11/114),the difference was not singnificant(P=0.448).The positive rate of T1 was 14.8%(9/61),the positive rate of T2 was 13.9%(5/36),the positive rate of T3 and T4 was 14.7%(26/177),the difference was not singnificant(P=0.992).The positive rate of no lymph node metastasis was 16.7%(15/90),the positive rate of with lymph node metastasis was 13.6(25/184),the difference was not singnificant(P=0.498).The positive rate of stage ? was 16.0%(4/25),the positive rate of stage ? was 14.3%(5/35),the positive rate of stage ? and ? was 14.5%(31/214),the difference was not singnificant(P=0.978).The median survival time of HER-2 positive patients was 15 months and the median survival time of HER-2 negative patients was 31 months.One-year,two-year,three-year survival rate of the former were 70.0%,15.6%,0.0%.Which of the latter were 96.1%,73.3%,43.4%,the difference was statistically singnificant(P=0.000).Conclusion:The expression of HER-2 in SRC tissues was relation to the type of tumor differentiation.The expression of HER-2 in type ? and ? SRC was higher than that in type ? and ? SRC.The expression of HER-2 in tumor tissue is significantly related with the prognosis of patients.
