Backgroung:Binswanger disease, or subcortical arteriosclerotic encephalopathy, was reported by Otto Binswanger, and named by Alzheimer in 1898. The main clinical symptom of BD is hypertension in old people, progressive mental deterioration and recurrent stroke. So far, no effective treatment is available for patients with BD. The quality of life in patients with BD is dramatically reduced, attributing to the progressive mental deterioration and the impaired ability of daily life. Not only that, to care and treat the patients is an incubus on the family and society. BD has been recognized with increasing frequency by high-resolution CT and especially MRI which can demonstrate slight white mater abnormalities. So, it is imminent to explore an effective treatment, which could improve the cognition and ability of daily life.Objective:To evaluate the effects of comprehensive nursing intervention on the daily life and cognition in patients with Binswanger disease.Methodology:Patients with Binswanger disease were sequentially enrolled into two groups:group A (n=32) with comprehensive nursing and basic nursing; group B (n=31) with basic nursing only. All subjects were tested for cognitive and functional abilities at baseline, at the third and sixth month after the beginning of the study.Main Results:There was a significant improvement in mimi-mental state examination (MMSE) score (p<0.01), AD assessment scale-cognitive (ADAS-cog) subscale (p<0.01), abilities of daily life (ADL) (p<0.01), and Barthel index (p<0.01) after comprehensive nursing intervention in group A, while no change was found in group B. Additional comparison was carried out to detect the degree of improvement in patients received comprehensive nursing intervention. Group A showed a greater improvement than group B in score of MMSE (p<0.01), ADAS-cog(p<0.05), ADL(p<0.01), Barthel index(p<0.01).Conclusions:Great benefits could be achieved on cognition and daily life in patients with Binswanger disease. |
