Research Background:Hyperplasia of mammary glands, a commonly-encountered breast disease in clinic, is associated with the occurrence of breast cancers to a certain extent; it pertains to the category of TCM "breast nodules" and breast pain and lumps are regarded as its major clinical manifestations. Incidence of this disease rises in our country year by year. Therefore, the clinic always pays more attention to the treatment of hyperplasia of mammary glands. Traditional Chinese medicine in treating this disease has better therapeutic effects, but it is difficult for patients to insist on taking Chinese herbs for a long time due to its long course; breasts are located on the body surface, so drugs for external application easily exert the effect. In addition, they have such advantages as convenient in use, insignificant toxicity and side effects and easy to insist on administration compared with drugs for oral administration. Thereby, hyperplasia of mammary glands refers to a kind of disease which can be achieved therapeutic purpose by application of external therapy.Research objective:This subject treats hyperplasia of mammary glands through external application of Ru'an gel paste and explores clinical therapeutic effects of external application of Chinese herbal preparation-Ru'an gel paste in curing hyperplasia of mammary glands by observing changes of clinical manifestations like breast tenderness, pain and so forth before and after treatment as well as variations of lump size.Research methods:59 patients with hyperplasia of mammary glands from breast special outpatient of the First Hospital Affiliated to Nanjing University of Chinese Medicine were selected and randomly divided into 30 cases in control group and 29 cases in treatment group. Ruzengning was taken orally in control group, whereas Ru'an gel paste was for external application to treat in treatment group. One menstrual cycle was considered as one course of treatment, two courses of treatment in total. Changes of clinical manifestations like breast tenderness, pain and so forth before and after treatment as well as variations of lump size were observed and compared in both groups.Research results:(1) both external application of Ru'an gel paste and oral administration of Ruzengning had certain therapeutic effects on hyperplasia of mammary glands. Total effective rates of treatment group (external application of Ru'an gel paste) and control group (oral administration of Ruzengning) were 96.6%and 93.3%respectively. Compared with control group, the total effective rate of treatment group had no significant difference (P>0.05), suggesting that therapeutic effects of external application of Ru'an gel paste and oral administration of Ruzengning have no distinct difference in treating hyperplasia of mammary glands, and clinical therapeutic effects of Ru'an gel paste for external application do not surpass those of Ruzengning for oral administration. However, this research results demonstrated that external application of Ru'an gel paste had definite therapeutic effects in treating hyperplasia of mammary glands, which can provide a new approach to the treatment of hyperplasia of mammary glands. (2) External application of Ru'an gel paste can evidently alleviate or remove patients'breast tenderness, pain and lessen or eliminate breast lumps. Pain and tenderness were immediately relieved after patients usually used external application for one course of treatment. In aspects of breast pain and tenderness, the therapeutic effect of treatment group had no significant difference (P>0.05) compared with control group and therapeutic effects of both groups were approximate. In terms of improving diameters of breast lumps as well as their limits of integration, treatment group had conspicuous difference (P<0.05) by comparison to control group. By analyzing variations of the diameters of breast lumps as well as their limits of integration before and after treatment, results presented that therapeutic effects of external application of Ru'an gel paste were not better than those of control group.Conclusion:Ru'an gel paste with functions of warming yang to disperse masses as well as promoting blood circulation and regulating qi to relieve pain has better therapeutic effects to alleviate clinical symptoms of patients like breast pain, hyperplasia of mammary glands, lumps and so on. Its external application with convenient method, insignificant toxicity and side effects as well as good patients'compliance has vast potential for future clinical application.
