Objective:By detecting local cytokines of Vaginal fluid between bacterial vaginosis (BV) and vulvovaginal candidiasis (VVC), analysis the immune regulatory mechanisms of cytokines in the lower genital tract infection and the effect of cytokines in disease and defense mechanism.Used ROC curve to evaluate the clinical diagnostic value of cytokines.Method:1, select the healthy group, detected cytokines by ELISA menstrual period, CD4+ /CD8+levels of VLF was detected by flow cytometry, peripheral hormone levels was detected by chemiluminescent immunoassay, analysis immune mechanisms of menstruation changes.2, divided into three groups to collect vaginal fluid:(1) 35 healthy women of reproductive age as controls. (2) bacterial vaginosis 35 women of childbearing age. (3) 35 vulvovaginal candidiasis women of childbearing age. Detected vaginal fluid cytokines of three groups by ELISA, IL-2, IL-8, IFN-y, IL-4, IL-13, IL-10, and IgE levels.3, through the ROC curve to evaluate the statistical side the diagnostic value of cytokines and to establish diagnostic points.Results:1, hormones and CD4+/CD8+ratio cyclical changed with the menstrual cycle,. CD4+/CD8+and hormones were negatively correlated. Follicular phase and the IL-8, IFN-y was negatively correlated. Age and IL-8 was negatively correlated. Age and IL-10 were positively correlated.2, the bacterial vaginosis group of IL-2 was significantly decreased, IL-13, IL-4, IL-10 significantly increased. Statistically significant (P<0.05) IL-8, IFN-y, IgE was no significant difference. Vulvovaginal candidiasis group of IFN-y were significantly decreased, IL-2, IL-13, IL-4, IgE has significantly increased. Statistically significant (P<0.05), IL-8, IL-10 no significant difference.3, ROC curve analysis, IL-2/IL-10 and IFN-y/IL-13 as a new variable to show a better specificity and sensitivity. Youden value method used to establish the diagnosis points of two disease.Conclusion: Influence of estrogen on the local immunity, suppresses T cell immune responses. Menstrual period and age-related cytokines relevant. Results show that the local immune system and the menstrual period and age. Thl/Th2 imbalance exists in these two diseases. Not increase of IL-8 and the increase of IL-13 is the main characteristics of patients with BV. Reduction of IFN-y and IL-4, increase of IL-13 is the the main features of patients with VVC, IgE elevation that there was local hypersensitivity in VVC. Because of local Thl/Th2 immune response is a dynamic process, ROC curve analysis found that the cytokines can not be a unilateral interpretation of the whole, the diagnostic value of two variables into a bigger, better reflect the characteristics of the immune system. Analysis of two local cytokine changes in disease can help to better control the infection.
