Study On The Influence Of Nursing Intervention On Uncertainty In Illness Of Postoperative Inpatients With Gastric Carcinoma

Objectives:(1)To identify the status of uncertainty in illness and explore the related factors of MUIS among patients with gastric carcinoma;(2)To examine the relationship among level of MUIS in patients with gastric carcinoma;(3)To explore health education impact on the status of psychology and MUIS of gastric carcinoma.Methods:A descriptive design and a quasi-experiment design were used in this study. From November/2009 to November/2010,130 patients of gastric cancer in the gynecologic ward of Tianjin Medical University Cancer Institute and hospital. All subjects completed the common information questionnaire, MUIS Questionnaire and Social support.At the same time, the value of systolic pressure(SBP), diastolic pressure(DBP), heart rate(HR)and lymphocyte(LYMPH) product tested in the same month was collected. All the data collected were evaluated:the status of MUIS and social support were described. The correlation among level of MUIS and social support was analyzed. In the part of quasi-experiment,130 subjects admitted were randomly selected into experimental group and control group by the number of rooms. Patients in the experimental group were given health education by the researcher and routine nursing. Patients in control group only received routine nursing. The duration of intervention was 6 month. Questionnaires and physiological values were collected again just after intervention. Paired t-test was used in comparing the status of MUIS, psychology and physiological values before and after intervention. Independent t-test was used in comparing between experimental group and control group.Result:(1) Surveyed 130 patients with gastric cancer postoperative hospital, their uncertainty in illness score 59-95, average at 77.84±12.86, in the middle degree of MUIS.(2) The single factor analysis of variance and t-test results showed that uncertainty of illness in postoperative gastric cancer patients is related with education, whether the job, occupation, residence and income related.(3) The average score of social support in gastric cancer patients is 38.47, higher than the norm. The score of objective to support is 8.38, the score of subjective support is 24.45, and the score of support for the utilization is 6.12. It does can be considered that postoperative gastric cancer patients'social support score higher than the population average.(4) The Pearson correlation analysis showed that uncertainty in illness postoperative gastric cancer patients-uncertainty dimensions of the utilization and support, support scores were negatively correlated (P<0.05), the remaining uncertainty of the dimensions and total score of disease social support was no significant correlation (P>0.05).(5) After intervention, the intervention group patients overall MUIS scores after the intervention than the control group scored lower, and statistically significant (P<0.05), indicating that the experimental group of nursing intervention on the disease in patients with reduced uncertainty has played a positive effect.(6) After intervention, the experimental group social support scored 41.62±6.58, and the control group social support scored 36.56±6.56, with statistical difference (P <0.05), show that the experimental group of nursing intervention on social support systems play a positive role in improving the social support.Conclusions:The uncertainty of research could help medical staff understand the needs of patients and their families and the emotional response. This study found that postoperative gastric cancer patients the level of uncertainty of illness is closely related with social support. Improve the patient's social support can reduce the level of uncertainty in illness. The experimental group of nursing intervention on patients to improve the social support system has played a positive role.
