The medical-service informatization is an international development trend, it speeds up the construction process of domestic hospital informatization. Clinical information system is an important part of hospital informatization, whose purpose is to improve the quality of medical treatment. At present, domestic clinical information system is on the stage of development, there are many business functions needing to be perfected, therefore, clinical information system must have high expansibility and maintainability.According to the scalability requirements and the application environment of the clinical information system, this paper proposed the distributed three-layer C/S/S structure system solution which based on COM+ component technology. This solution packaged the database access and business logic as COM+ component on the application server, so it improved the system scalability and reduced the development cost of subsystem. Moreover, the idea of software upgrade automatically was used in Clinical information system, it achieved the client automatic updates and met the system requirements of easy maintenance.Firstly, this paper has analyzed the theory of system architecture, COM+ component technology and .NET Remoting technology. Secondly, a detailed demand analysis of clinical information system is carried out through UML and specific objectives are made. In the system outline design stage, this paper proposed system architecture design principles and overall system architecture, in addition, it chose the system platform, and then it designed system functions and CDM of the database. In the system detailed design stage, this paper gave the design principles of the system and the detailed design of the data access layer and business logic. Lastly, we achieved the system database access and business logic by the COM+ component technology and achieved distributed communications of the system which combined with .NET Remoting technology.This system has been running officially, practice has proved the three-layer C/S/S structure application of clinical information system based on COM+ component technology enhanced the system expansibility; In addition, the client automatic updates solved version control and maintenance problems efficiently and reduced the system maintenance work and enhanced the working efficiency.
