| Objective To explore the risk factors of periodontitis in patient with essential hypertension and provide the scientific basis preventing and contorling them.Methods A case-control study was conducted among 245 patients with essential hypertension, including 155 patients with periodontitis and 90 without periodontitis,which they all came from medical clinical and were diagnosed as having an essential hypertension. All studies were investigated face-to-face and clinical examination.Their sex,age,hypertension histroy,smoking,hypertension complications, blood pressure,oral hygiene instruction and health instruction were recorded. Plaque index,gingival index,probing depth,loss teeth,attachment loss were included in oral examination.Data was analysed with unconditional logistic regression to investigate the related factors of periodontitis in patients with essential hypertension.Results Univariate unconditional logistic regression analysis showed that there was a significant difference of plaque index,smoking,body mass index,gingival index,the course of hypertention,hypertention,oral hygiene instruction and oral instruction in patients with essential hypertension complicated periodontitis (P<0.05), The odds ratio OR and OR 95%CI were7.127(3.789-13.371),3.654(1.995-6.692),2.053(1.207-3.491),3.990(2.230-7.140), 0.978(0.885-1.081),2.616(1.391-4.922),0.321(0.186-0.553),0.364(0.213-0.624). In multiple logistic regression,body mass index,smoking,plaque index,gingival index,oral hygiene instruction and oral instruction showed significantly independent association with periodontitis (P<0.05), The odds ratio OR and OR 95%CI were 2.383(1.224-4.641),4.012(1.961-8.206), 5.424(2.604-11.301),3.002(1.484-6.075),0.367(0.185-0.726),0.368(0.186-0.728).the protect factors were oral hygiene instruction and oral health instruction.Compared with mild periodontitis in patients with essential hypertension, moderate and severe periodontitis also had relation to the course of hypertension and hypertension in Univariate unconditional logistic regression analysis (P<0.05), The odds ratio OR and OR 95%CI were 1.014(0.908-1.133), 4.205(2.110-8.378).In multiple logistic regression, moderate and severe periodontitis also had relation to hypertension.The odds ratio OR and OR 95%CI were 2.990(1.130-7.909).Conclusion we should strengthen oral health instruction and oral hygiene instruction including giving up smoking,controling hypertension and body mass index,so that prevent and contorl periodontitis in patients with essential hypertension and improve life for patients with essential hypertension. |
