Objective:Atlantoaxial dislocation of the clinical classification system TOI dynamic type system study to compare the different inter-observer reliability and reproducibility within the same observer.Methods:By 5 or more titles with the subtropical high and the composition of the expert spinal surgeons a full range of 50 cases with clinical data, using TOI classification system in the independent, double-blind, randomized state were assessed and classified.A month later, then repeat the same procedure the expert classification.Each expert had any cases of measurement data can not be left any mark.Collection of results, respectively, for the credibility and among different observers within the same observer reproducibility of the analysis, calculation ofκvalues, test consistency, the two classification systems were compared.Results: TOI classification system reliability and repeatability of the overall average is 83.97%(κ=0.787) and 88.20%(κ=0.842). Type T1, the overall reliability and repeatability were an average of 93.46%(κ=0.849)and 93.33%(κ=0.851)); T2 the overall reliability and repeatability were an average of 92.69%(κ=0.825)and 93.33%(κ=0.837); O type of the overall reliability and repeatability were an average of 94.32%(κ=0.861)and 94.87%(κ=0.883); I type the whole Reliability and repeatability are the average 95.51%(κ=0.893)and 95.90%(κ=0.905).Conclusion:1.TOI T2 classification system had a higher classification reliability and repeatability, but the T1 and T2 O higher than the reliability and repeatability, while I type with the highest credibility and repeatability.2.TOI classification system in a more comprehensive classification system overall reliability and reproducible, the sub-type of the atlantoaxial dislocation and its efficacy evaluation of a clinical guide.
