A Study On The Correlation Between Surgical Treatment Of Atlantoaxial Dislocation And Supplementing Temperature And Yang

Objective:The upper cervical spine has special anatomical structure and complex functions.Once the injury and disease occur,cervical spinal cord sensation and dysfunction may occur,which may also be manifested as clinical manifestations of central nervous system damage,while atlantoaxial dislocation or occipital instability may be directly Compression of the upper cervical medulla-respiratory center,life-threatening.This article reviews the history of atlantoaxial dislocation classification,the progress of related surgical treatment,the understanding of TCM on the diseases related to the dislocation,the pathogenesis of Du Meridian,the treatment principle and clinical efficacy.Under the guidance of the integrated diagnosis and treatment of spinal diseases,the improvement of symptoms before and after surgical treatment of patients with atlantoaxial dislocation and the corresponding symptoms of traditional Chinese medicine were studied to investigate the correlation between the treatment of AAD and invigorating Qi and warming YangMethods:From June 2016 to March 2019,16 patients with atlantoaxial dislocation in the Center of Upper Cervical Spondylopathy,China Japanese Friendship Hospital,including 9 males and 7 females,were treated with corresponding traction,surgical reduction and fusion fixation according to TOI classification.After operation,the patients were strictly immobilized for 12 weeks and followed up for 3 to 6 months.Statistical methods were used to compare SAC,JOA and score of Qi deficiency syndrome before and after treatment in each group.Results:The SAC in T,O and I patients was 10.31±4.23 before treatment,16.08±2.69 at the last follow-up,P<0.05;the scores of Qi deficiency syndrome in each group were 13.13±3.59 before treatment,19.19±2.81 at the last follow-up,P<0.05;the JOA scores in each group were 8.5±3.51 before treatment and 12.83±2.32 at the last follow-up,P<0.05.Conclution:Professor Tan Mingsheng took the lead in reporting the treatment of atlantoaxial dislocation with atlantoaxial pedicle screw internal fixation technology in 2002.At present,atlantoaxial pedicle screw internal fixation technology has been widely used throughout the country and even around the world,and is known as the "golden standard"of surgical treatment of AAD in upper cervical spine surgery.On the basis of literature records and modern research,the diagnostic criteria and integral of Du Meridian Stasis Syndrome are preliminarily put forward.For the treatment,a comprehensive treatment plan is put forward,which includes first operation,post-operation cooperation with Buyang Huanwu Decoction and Du Meridian Electro-acupuncture,and taking Buyang Huanwu Decoction for reinforcing Qi and blood stasis,to carry out"decompression of spinal cord and dredging Du Meridian".So far,the applicability of the comprehensive treatment of atlantoaxial dislocation"decompression of the spinal cord,dredging the Du meridian"has been further verified by clinical observation and good early clinical observation results have been achieved.The results show that surgical spinal cord decompression combined with Buyang Huanwu Decoction and Du meridian electro-acupuncture can restore the motor energy of the spinal cord,improve the integral of stasis of the Du meridian and collateral pain and other clinical symptoms.It is superior to single therapy.The experimental study confirmed that Buyang Huanwu Decoction and Dumai Electro-Acupuncture could repair spinal cord motor function in rats by inhibiting apoptosis and microthrombosis.For patients with atlantoaxial dislocation,TCM usually classifies the clinical symptoms of neck and shoulder pain,limb numbness and other clinical manifestations as blockage of blood stasis and obstruction of meridians and collaterals.The treatment mainly focuses on promoting blood circulation and removing blood stasis,and achieves favourable results.From the pharamacological study of modern Chinese medicines and prescriptions,Chinese medicines or prescriptions with functions of activating blood circulation and removing blood stasis can promote spinal cord repair and have definite spinal cord protection.Du Meridian is the sea of Yang pulse,which accumulates and regulates Yang Qi of all body,and the relationship between Qi and blood is close.Blood can not be separated from Qi.The results of this paper suggest that individualized treatments such as traction,anterior and posterior surgical reduction and fusion fixation for each type of atlantoaxial dislocation according to TOI classification can significantly improve the clinical symptoms of nerve compression damage and effectively alleviate the corresponding symptoms of Du Meridian Yang Qi deficiency syndrome.The results show that surgical treatment of atlantoaxial dislocation patients according to TOI classification can achieve the goal of invigorating Qi and warming Yang in TCM.There is a close correlation between surgical treatment of atlantoaxial dislocation and the principle of invigorating Qi and warming Yang in TCM.
