Objective:To research on the correlation between surgical treatment for atlantoaxial dislocation and the theory of " Taking Both Motion and Quiescence Into Consideration".Method:Between September2004to June2011,59cases suffering from atlantoaxial dislocation under going atlantoaxial pedicle screw instrumentation were reviewed retrospectively. There were33males and26females with the mean age of45.8±15.8years (ranged,8to72years). All patients presented with limitation of activity of cervical vertebra, and with pain at craniocervical, junction and neck stiffness. Standard anterior-posterior,and flexion-extension radiographs, computed tomography and MRI of cervical spine were obtained in all patients before operation. All patients presented with atlantoaxial dislocation. The JOA score and the activities were used to evaluate the surgical outcome. Results:The JOA score before treatment and the last follow-up were (9.7±2.3)、(15.4±1.3) respectively; The activities of cervical antexion, extension, left lateral flexion, right lateral flexion, left rotation, right rotation were (41.81±9.83)、(41.64±11.25)、(35.99±6.38)、(36.40±6.89)、(61.02±12.26)、(62.71±11.26)"respectively, the last follow-up of (48.81±8.07)、(48.52±7.87)、(44.33±5.37)、(44.00±5.48)、(74.33±7.27)、(74.29±6.14)°.JOA score and the activities of cervical significantly improved compared with that of before treatment. Conclusion:Atlantoaxial dislocation treated by by atlantoaxial pedicle screw instrumentation, can effectively relieve the nerve function and improve the activities of cervical, and how that operation treatment is closely related with the principle of "the Combination of Movement and Immobilization".
