Psoriasis(Ps)is a chronic inflammatory skin diseases with the erythema,crumbs as the main performance.The pathogenesis is complex and hasn't yet be explained.The diseases is recurrent,but there isn't effective means and drugs to treat.There is not any breakthrough in the therapy to PS yet.Although the modern medication has a satisfying curative effect in the near future, it cannot be long-termused for its toxicity and side effect.What's more, recurrence and aggravation could occur after drug withdrawal.However, base on holistic concept,Chinese medicine has positive effect but little side effect on Ps.Chinese medicine treatment of psoriasis using the overall concept of Chinese medicine,syndrome differentiation and integration with oral drug therapy and uphold the positive effect of treatment.The"Xiaoyinjiedu Drink"is from the experience prescription"Xiaoyinjiedu Soup"of the famous old Chinese medicine professor Jin Qifeng.The clinical application of the drug has been for many years and the efficacy is certain.In this paper,we have studied the clinical efficacy of the"XiaoyinjieduDrink"for psoriasis patients with hot blood and the regulation for the T lymphocytes and the co-stimulatory factor of their peripheral blood.Literature reviews and research report are the main components in this paper.Two literature reviews are included.The comprehensive and systematic review of progress in psoriasis research base on the comprehensive collation and analysis for the collection of Chinese and Western medicine in a large number of psoriasis-related literature.The first reviews the research of Chinese medicine for psoriasis on the Etiology,Pathogenesis and Treatment through the traditional Chinese medicine and modern medicine both in understanding and research.The second reviews the relationship between the T cells and the pathogenesis of psoriasis through the T cell activation,peripheral blood and skin lesions of the T cells,T cells and keratinocytes,as well as the role of inter-B7-CD28/CTLA-4 research in Psoriasis.This report is divided into two parts.One is about clinical research, and the other is about experimental one.In the clinical research,30 patients with psoriasis take xiaoyinjiedu drink for two courses of treatment.Respectively,score the lesions,the efficacy and the TCM syndrome,investigate the clinical efficacy of the xiaoyinjiedu drink for the psoriasis.Results:After acourse of treatment,the PASI score is significantly different from that before treatment(p<0.05).On the Determine of effect,the cure rate is 6.6%,the markedly effective rate is 46.7%,the total effective rate is 100%.On the TCM symptom score,there is no significant difference between after a course and before treatment on stool,urine and pulse(p>0.05).There are significant differences between after a course and before treatment on Itching,dry mouth and tongue(p<0.0t).The TCM total symptom score compares with those before treatment is significantly higher(p<0.01).After two courses of treatment,the PASI score is significantly different from that before treatment(p<0.01), On the Determine of effect,the cure rate is 13.3%,the markedly effective rate is 60%,the total effective rate is 100%.On the TCM symptom score, there is no significant difference between after a course and before treatment on stoot(p>0.05),There are significant differences between after a course and before treatment on urine,pulse,Itching,dry mouth and tongue(p<0.05).In the experimental research,through the detection of the expression of T cells and the co-stimulatory factor in the peripheral blood of 30 psoriasis patients and 20 healthy volunteers by the flow cytometry,explore the differences about the expression of the T cells and the co-stimulatory factor in peripheral blood of psoriasis patients and normal,discuss the regulation of the xiaoyinjiedu drink to T cells and co-stimulatory factor.Results:Before and after treatment the CD3~+,CD4~+ cells and CD4~+/CD8~+ ratio of the patients decrease(p<0.05)and CD8~+ cells of the patients increase compared with those in normal(p<0.05).The CD28 and its fluorescence intensity before treatment were higher than the normal group, and after treatment the number of CD28 positive cells in the normal group was higher than that after treatment,the fluorescence intensity was significantly lower than normal after treatment.The CD86 and its fluorescence intensity before and after treatment were significantly higher than the normal group(p<0.05).The CD80 and CD80 fluorescence intensity of the normal group and before treatment have no significant statistical difference(p>0.05),but the normal group there is a downward trend.There is no clearly significant difference about the T-cell subsets CD3~+,CD4~+,CD8~+,CD4~+/CD8~+ ratio and the co-stimulatory factor CD28,CD28 fluorescence intensity,CD80,CD80 fluorescence intensity,CD86,CD86 fluorescence intensity in 30 patient's peripheral blood before and after treatment(P>0.05).The study have shown that xiaoyinjiedu drink have significant clinical efficacy for the psoriasis with blood heat,the expression of T cell subsets and co-stimulatory factor in the psoriasis patients'peripheral blood compared with the normal.The experimental results don't show that the xiaoyinjiedu drink has a positive role in immune regulation for the psoriasis,also don't have shown that parallel correlation between clinical symptoms in patients with psoriasis and their immune status exists.The regulating role of the xiaoyinjiedu drink for the body's immune need to be further studied.
