Symptoms And Stages, Pancreatic Cancer Study On The Correlativity Between Research And Therapy Of Traditional Chinese Medicine

This dissertation is composed of three parts:a review article,a retrospective analysis and a clinical research.The first part is a review article,which includes the main contents as following:the characteristics of pancreatic cancer,the main principles and methods of treatment in both China and abroad in recent years.The methods of treatment include surgical therapy,radiotherapy,chemotherapy,minimally invasive therapy,target therapy,TCM therapy and other therapies.The second part is a retrospective analysis of symptoms in 111 patients of pancreatic cancer.Objective:To summarize the symptoms of pancreatic cancer clinically,study on the relevance between symptoms and staging,symptoms and sites,in order to provide theoretical basis for treatment of pancreatic cancer by using TCM recipe or herbs rationally.Methods:We summarized the symptoms, the stages and the sites in 111 patients of pancreatic cancer,studied on whether there was relevance between symptoms and stages,symptoms and sites.Results: The first symptoms started with epigastric discomfort,upper abdominal pain, loss of appetite,weight loss,low back pain,fatigue,jaundice,diarrhea, shoulder pain in most patients.The common medical symptoms is upper abdominal pain,weight loss,epigastric discomfort,loss of appetite,jaundice, constipation,fatigue,shoulder pain,low back pain,nausea and vomiting, diarrhea.Among which only jaundice in patients with the head pancreatic cancer was significantly higher than the body and tail of pancreas cancer,the rest symptoms in different stages and different parts of the tumor had no difference. Conclusion:Common symptoms of pancreatic cancer patients were:upper discomfort, upper abdominal pain,loss of appetite,jaundice,weight loss,low back pain, shoulder pain,fatigue,constipation,or course of disease often diarrhea, abdominal mass,dry mouth,nausea and vomiting,fever,etc.Jaundice which was reflected in patients with the head of pancreatic cancer was significantly higher than the body and tail of pancreas cancer,and the other symptoms and tumor stage, tumor site has no relevance.The main clinical manifestations of pancreatic cancer is similar with the "Treatise on Febrile Diseases-Part Jueyin disease" symptoms,consideration could be given from the treatment of pancreatic disease Jueyin.The third part is a clinical study.Objective:To observe the treatment of pancreatic cancer with the ebony pill addition and subtraction to improve the degree of clinical symptoms and clinical benefit rate,the focus of observation of the pain and appetite to improve the situation.Methods:21 patients,who were consistent with the eligibility criteria of the clinical study,given the decoction of Chinese medicine orally;the main prescription is the ebony pill addition and subtraction,combined with external use of herbs Wenyang Huatan Ranjian Sanjie.Patients were observed at 3,6,9,12 days,and monitored for pain, loss of appetite.Three months later,the clinical symptoms were examined with a comprehensive score,on pain relief,appetite,and changes in their quality of life,in order to determine the clinical efficacy of the treatment with the ebony pill addition and subtraction.Results:21 cases of pancreatic cancer with symptoms of an integrated average score of 25.43±7.62 points and 13.81±6.38 points(before and after the treatment,respectively),P=0.0006,had significant difference.After 14 days treatment,the total remission rate of pain is 52.63%,The shortest time of the pain ease was 2 days(2 cases).Loss of appetite symptoms improve significantly,the fastest time to improve the appetite is two days(1 case).Before and after treatment,the KSP of patients was significantly improved.Clinical benefit rate is 71.43 percent.The median survival time of the 21 patients was 7.0 months and the average survival time is 5.98±0.29 months.Conclusion:The pancreatic cancer can be considered from the disease of typhoid Jueyin governance.The ebony pill addition and subtraction has a significant effect in the treatment of pancreatic cancer,patients can benefit from it.The next step we are prepared to carry out a large sample clinical observation,in order to facilitate the further promotion of treatment.
