| Objective:This thesis studies the relationship between hemagglutination and traditionalChinese medicine tagnation of blood by observing the therapeutic effects ofChuanxiongzine on blood rheology, patients with stagnation of blood symptom aftertreatment by Chuanqing and other related index in CRF patients with stagnation ofblood, to seek the modern iatric evidence for the traditional Chinese medicine'stherapeutics for CRF from the angle of stagnated blood.Methods:1. The 45 cases come from patients with CRF from Wuhan No.1 hospital during2005.12 to 2007.4 who were diagnosed stagnation of blood, the renal functionabove stage of exhaustion, haven't got substituted treatment and hospitalizationexceeding 15 days.2.To randomly divide the 45 patients into two groups: treatment group (23 cases) andcontrol group (22cases).3.To divide the 45 patients into light, middle and heavy fraction mark according to theappearance of stagnation of blood which were defined by the principle.4.Two groups both take symptomatic treatment, such as anti-infection, depressurization, correcting electrolyte disturbances and acid-base imbalance. Onthe basis of former treatment, treatment group additionally use Chuanxiongzine160mg combined with 5% Glucose 100mL(those who has high blood sugaraccepted sodium chloride 100 mL)Ⅳdrop, once a day, use 15 days like this.During stage of therapy, do not use decoagulant, anti-platelet aggregation drugs, lowering fat drugs and ACEI.5.To observe the changed states of blood theology after treatment.6.To compare the changed states of stagnation of blood after treatment.7.Other biochemistry index: Scr, BUN, Ccr, Alb, HGB, 24 hours Urine proteinquantitation, ESR, Fib, urine FDP, TC and TG.8.To compare the associativity between different renal function stage andaccumulated points of stagnation of blood symptom. Rerults:1.The score of stagnation of blood symptom is 4.71±0.87 in compensation stage ofrenal function, 8.63±1.14 in azotemia stage, and 10.81±1.65 instage of exhaustion.2.The total effective rate of stagnation of blood of treatment group is 91.3%, thecontrol group's is 59.1%, these is a significant difference between two sets(P＜0.025).3.The whole blood viscosity under high shear rate, whole blood viscosity under lowshear rate, HCT, blood plasma viscosity under high shear rate and Erythrocyteaggregation index number of treatment group have decreased after treatment, thereis a significant difference compared with the control group's (P＜0.05 or P＜0.01).The red blood cell deformed index number has upgraded after treatment, andthere is a significant difference compared with the control group's after treatment(P＜0.05).4.The ESR, blood plasma Fib, urine FDP of treatment group all have decreased aftertreatment, and compared with the control group have significant difference (P＜0.05or P＜0.01); the TC and TG of two sets patients has no significant difference aftertreatment (P＞0.05).5.The Scr and BUN of treatment group have decreased significantly after treatment(P＜0.05 or P＜0.01), Ccr has raised up significantly after treatment (P＜0.05); thecontrol group just has BUN degression(P＜0.05)after treatment. Compared with thecontrol group, the Scr, BUN and Ccr meliorative states of treatment group is betterthan the control group's after treatment (P＜0.05). There is no significant differencebetween the two sets patients on blood Alb, HGB after treatment (P＞0.05).6.The 24 hours Urine protein amount of two sets have significantly degraded aftertreatment(P＜0.05), but has no significant difference between two sets (P＞0.05).Conclusion:1.The score of stagnation of blood symptom raises up follows renal functiondegrading.2.Chuanxiongzine can improve stagnation of blood symptom of the patients withstagnation of blood. 3.Chuanxiongzine can improve blood rheology index significantly.4.Chuanxiongzine can depress ESR, Fib, FPD significantly, has upstanding effects toadjust cruor-dissolve system mussy.5.Chuanxiongzine can depress blood Cr and BUN, raise Ccr significantly, but whatkind of influence on the renal function long-term hasn't definited yet. |
