Objective:Primary glaucoma is a group of disease which has a comm.- on feature of optic atrophy and visual field defect. It has been arranged for the second place and the blinding process is hidden and irreversible. However,due to the mechanism of its cause has not yet to clear and the mechanism of damage has not been completely clarified, the mechanism of optic nerve damage in glaucoma has two main doctrine,that is mechanical theory and ischemic theory.But many patients damage of optic function keep developing into optic atrophy even their IOP has been controlled. Another part of the normal-tension glaucoma patients with intraocular pressure in normal range,it occurred in a typical glaucoma optic atrophy and visual field defect.This shows that the occurrence of intraocular pressure in glaucoma is not the only risk factor. Ischemic theory is more and more attentioned.This study attempts to learn from the the angle of hemor- heology,detect hemorrheology Change in patients of primary glaucoma. United Color Doppler imaging (color Doppler imaging, CDI) Detect the blood flow parameters of ophthalmic artery (OA)and central retinal artery (CRA) at the same time to explore the pathogenesis related factors of primary glaucoma to provide reference of the primary glaucoma clinical diagnosis and treatment .Methods:The blood rheology indices (including blood viscosity, plasma viscosity and fibrinogen ) were determined in primary open angle glaucoma (POAG) patients,acute angle closure glaucoma (AACG) patients, chronic angle closure glaucoma (PCACG) patients,20 cases of normal subjects chosen as control group (patients with senile cataract), 20 cases every team ophthalmic artery (ophthalmic artery,OA) and Central artery of retina (central artery retina,CRA) were detected in glaucoma group (60 cases ) and control group (20 cases) .The indicators of detection were the peak systolic velocity(peak systolic velocity,PSV),end diastolic velocity (end diastolic velocity,EDV),resistant index(resistant index ,RI), the shape of spectra and the frequency of monitor. Patients and the control group were taken to a strict screening criterion, age and sex were matched. Comparing to the differences of each group with T test.Giving a linearly related to analysis between the blood viscosity target and CDI blood flow target with primary glaucoma patients.Results:1.Determination of blood rheologyThe blood rheology indices in POAG group were significantly higher than those in the control group(P<0.01).The blood viscosity and plasma viscosity in primary angle-closure glaucoma (PACG) group were signi- ficantly higher than those in the control group(P<0.05).There was no significant difference for the fibrinogen between PACG and the control group (P>0.05).The blood rheology indices in POAG group were signify- cantly higher than those of PACG group (P<0.05).There was no difference for the blood rheology indices between PAACG and PCACG group.2.Determination of hemodynamicCompared with the control group.PSVand EDV were significantly decreased.and RI was remarkably increased ( P< 0.01) in 2 arteries in patients group.There were no statistical differences in 2 hemodynamic parameters between ACAG and CCAG groups.but EDV was decreased,and RI increased obviously (P<0.05) in 3 arteries of POAG subjects in comparison with PAACG and PCACG patients .Hemodynamic parameters in glaucoma patients were characterized as higher resistance and lower velocity.3.the Correlation Analysis between the the blood viscosity target and CDI blood flow targetThe all targets levels of blood viscosity in PG patients were significantly negatively correlated with the PSV,EDV of both the OA and CRA (P<0.05),and were significantly positively correlated with the RI of OA and CRA (P<0.05) ;There were no significant correlations between the levels of blood viscosity and the parameters of ocular hemodynamics in the normal control (P> 0.05).Conclusion:The results suggest that both POAG and PACG patients have blood hyperviseosity which is a potential risk factor of glaucoma.The blood hyperviscosity plays a more important role in the pathegenesis of POAG than in PACG. The hyperviscosity may have similar effect in pathogenesis of PAACG and PCACG. There were regional circulatory disorders in different types of glaucoma .CDI is sensitive to detect hemo- dynamic change of the optical papilla,which is valuable in the early diagnosis,monitoring medication response and predicting prognoses is in the patients with glaucoma.
