| Retarded Menstruation (RM) is a commonly and frequently occurring disease in the clinic disease of gynecology. It causes this disease difficult to cure even after long treatment that several types of disease factors interact altogether. There is a high occurring rate of RM among Singapore women. Patients usually pay visit to the doctor to get western medical treatment. However, it is not so effective. Through the observation of the treatment effectiveness for 70 cases of RM of Singapore women taken under Shanghai Chinese Medicine Center, it is highly expected to explore the effective treatment to the RM with the traditionally Chinese compounded method.The first description for this type disease in Traditional Chinese Medicine (TCM) history was observed in Han Dynasty's《Synopsis of Golden Chamber. Woman Compounded Disease Simultaneously Treatment for Vessel and Syndrome》. Zhang Zhong Jing, the author, described this disease as so called "menstruation not occurring at the timing", and cured it with Wen Jin Tang. The following ancient famous doctors have done a lot further studies in regard with the disease's root causes, disease's pathogenesis and its syndrome differentiation typing (SDT). The cure method applies orally taken traditional Chinese medicine (including TCM period, SDT, integration of traditional and western medicine (ITWM),(?) treatment by specialized prescription and empirical prescription and so on), the recent times famous TCM doctor invented a lot great ideas to cure this disease. Besides, there are many external therapies providing sufficient materials for modern treatment, such as ear acupuncture point pressing, acupuncture and moxibustion and massage. Meanwhile, further and deeper studies are carried out upon TCM treatment pathogenesis to RM, the collation with the western medicine's biochemical index and the BBT detection application in RM. The RM for Singapore women is mainly due to deficiency of kidney Qi and stagnancy of liver Qi. Its main pathological change parts are liver and kidney. Deficiency of kidney Qi is caused by born shortage of kidney Qi thus flush and conception vessel no fullness, leading to RM even after menarche, by great abundance and sexual overstrain thus damage to kidney essence and no essence converting to blood, postponing the fullness time for the blood sea and leading to RM, by kidney Qi asthenia, damage to Yang after accumulating long period thus viscera missing the YangQi's warming and graceful degradation, influencing the blood's producing and converting, flush and conception vessel no fullness and postponing the fullness time for the blood sea and finally by no sufficient kidney essence, damage to Yin after accumulating long period thus Yin deficiency causing fire, hot damage to the essence and blood and water deficiency and blood decease, leading to RM. Liver depression and Qi trapping is caused by temperament depression, uneasy self feelings, liver Qi depression, failure of catharsis, leading to RM and by Qi depression and blood trapping, lush and conception vessel trapping and obscure, resistance to blood moving and postponing the fullness time for the blood sea and leading to RM.The study refers to the "Instructing Principle of New Drug Clinical Research" and follows the section on treatment of irregular menstruation to establish the diagnosis and treatment criteria, the TCM syndrome differentiation criteria and the clinical symptoms grading quantization indexes for Retarded Menstruation with Syndrome of Deficiency of kidney and stagnation of liver Qi.. We observed the treatment effectiveness after using liver-regulating and kidney tonifying, blood-maintaining and menstruation-regulating method on 70 cases of retarded menstruatin patients. All 70 cases of RM of Singapore women were taken under Shanghai Chinese Medicine Center, with the patient's age from 16 to 43 years old. In details, there are 20 cases below 25, 23 cases between 25 and 35 and 27 cases above 35．There are 33 cases for unmarried patient, and 37 cases for married one. In terms of the disease history, the shortest one is 3 months, and the longest one is 15 years .In respect to the degree of disease seriousness, there are 25 cases in slight stage, 24 cases in moderate stage and 21 cases in severe stage. Of the disease symptom catalogue, all 70 patients are randomly sampled in sequence and equally divided into two groups so called as Traditional Chinese Medicine treatment (TCMT) and ATCMT. There are no obvious differences prior to the treatment of the two groups in regard with patient's particulars such as age, illness period, and illness degree of severity and illness symptom(p>0．05) . Therefore, the two groups can be compared in a meaningful manner. TCMT group adopts the pure Chinese herbs, targeting at regulating the live, tonifying the kidney, nourishing the blood and regulating the menstruation. Applying ease powder as the fundamental, the herbs are composed of radix bupleuri 10g, Chinese angelica 10g, white peony root 10g, rhizome of large-headed atractylodes 10g, poria cocos wolff lOg, dodder seed 12g, Himalayan teaselroot 12g, fructus ligustri lucidi 10g, nutgrass galingale 10g, peppermint 5g, liquorice root 5g. Herbs are added, according to the symptom: milkvetch root and dangshen for asthenia of qi and blood; pinellia tuberifera, dried tangerine peel and rhizome atractylodis for phlegm-damp obstructing blockade and cassia bark and herba epimedii. The medicine is taken twice daily one each in morning and evening, staring at the fifth day of the menstruation period with a full curing period till next menses. ATCMT group adopts same medicine as above and accompanies with the acupuncture. Take acupoint selection at TaiChong, uterus, point ex-b8, ShenShu, GanShu, GuanYuan, blood sea for smoothing liver and nourishing kidney and regulating the blood and restoring menstrual flow. Take associated acupoint selection at Zusanli and SanYinJiao for astenia of qi and blood; middle gaster and Zusanli and gate of life for blood cold. It takes twice a week, with the needle method of adoption reissue, which means to apply mountain burning manipulation after getting qi.For both groups of ATCMT and TCMT, the total treatment effectiveness are summarized at the time of tracing during three treatment courses and three months follow-up survey after treatment completing. The results show ATCMT 80% and TCMT 68．8% total effectiveness respectively. This also statistically illustrates the obvious superiority of the group by ATCMT over TCMT in treatment of the Retarded Menstruation (P<0．05) . Clinically, it also shows superior effectiveness under the group by ATCMT over TCMT in terms of the improvement of four types of main symptoms and seven types of associated symptoms of Retarded Menstruation P<0．01) , particularly for Shortage of Menstruation Amount, Blood Clot, Lumbosacral Pain, Nervous Tension under catalogue of main symptom and Physical and Mental Fatigue, Breast Swelling and Pain, Morbid Leucorrhoea Abnormality,Decline or Absence of sexual desire under catalogue of associated symptom (P<0．01) . In all, it is proven that the group by ATCMT enjoys the obvious superiority over TCMT in regard with the improvement on both the clinical symptom and body symptom. The results of comparing the treatment effectiveness by the two groups indicate that the higher the serious degree of the illness, the more difficult to cure, the longer of the illness history, the more uneasy to recover and the older of the patient, the worse of the treatment effectiveness. It indicates good effectiveness of clinically early treatment to RM, the earlier, the better.Through the observation on clinical treatment effectiveness for the 70 cases of Singapore woman's RM, this work shows that the method of regulating the liver and tonifying the kidney can effectively cure the Singapore woman's RM. The compounded method of ATCMT can enhance the cure effectiveness. |
