Objective: To compare and to assess the treatment of intertrochanteric fractures by proximal nail (PFN)and dynamic hip screw (DHS).Methods: From Map 2003 to September 2006, 64 cases with intertrochanteric hip fractures were followed up. They were treated with dynamic hip screw(DHS group A) and proximal femoral (DHS group B).The data of two groups were collected for satatistical analysis on the following aspects :operative time, length of incision time, blood loss, drainage,time of fracture clinical healing, postoperative complications , Sanders Hip score .Results: The differences of the operative time , blood loss, fluoroscopy exposures, drainage and clinical healing time between group A and group B were of significance (P<0.05). In comparing with group A and group B was of shorter operative time and clinical time , fewer blood loss and drainage ,but of more fluoroscopy exposures .The differences in post-operative complications between of group A and group B was of no statistical significance (P>0.05 ).Conclusion: By comparison , for type I A and I B angled plate ,DHS and PFN are all applicable. For type II and III ,PFN is suitable .For some cases of type II and III in which the outer cortical bone for insertion are intact, DHS is applicable ,The intertrochanteric fracture plane showing a coronal position can not use DHS, but PFN is appropriate .For reversed intertrochanteric fracture ,PFN is be used.
