The thesis on acute myocardial infarction (AMI) complicated by systemic inflammatory response syndrome (SIRS) consists of both the part of the introduction of concerned discourses and the part of exposition of the clinical research.[Review Articles]The first discourses, as being an overview concerning the progression of investigation on AMI in Traditional Chinese Medical Science(TCMS) ,includes the recognitions from history literature, the etiopathogenisis and pathogenesis of AMI, the determination of treatment based on pathogenesis obtained through differentiation of symptoms and signs, and the treatments in integrated traditional and western medicines in recent years. The second discourse deals with the recent achievement and development on SIRS study, which commends the concept , physiology and pathology clinical applications, clinical significance and the research in TCMS as well.[Clinical Research]Objective: To explore the treatment to AMI complicated by SIRS, to investigate the relationship of the syndrome between AMI and SIRS and its prognosis, to improve the recognition to the conception of SIRS, and to estimate the evaluation of SIRS in AMI by means of the statistic survey on the epidemiology of SIRS occured in the patients with AMI, the ways of studying the correlations between SIRS and the death during the initial course of AMI, and comparing with the variability between the SIRS and non SIRS in pathogenetic feature ,neopathy and prognosis, and the correlations of symptom in TCM, have been used.Methods: The clinical datas of 265 patients with AMI were collected and analysed. The worst datas of the 4 diagnostic items were recorded at the same time during the first week in hospital. Every patient who had at least 2 items of 4 diagnostic criteria such as fever, hypothermia, tachycardia, respiratory frequenc-y increase or leucocyte anomaly was decided as positive of SIRS.The epidemio logy, complication and death rate, heart function and prognosis were compared and analysed retrospectively between the SIRS group and non SIRS group,and the symptoms in TCM were compared as well.Results: the case fatality ratio during the first month is 18.1%.The morbidity of SIRS in the course of AMI is 38.4%.death rate: the SIRS group was 35.6%, non SIRS group 7.3%; that means the former is much higher than the later.
