ObjectiveTo study the epidemiological characteristics,clinical features and diagnosis and treatment of acute heart failure associated with acute myocardial infarction,80 hospitalized cases had been analyzed retrospect-ively to summarize the current status,which may provide reference for future clinical practice and research.MethodsThe subject collected the hospitalized cases of acute heart failure associated with acute myocardial infarction in the First Affiliated Hospital of Guangzhou Chinese Medicine University which coronary artery blood flow reconstructed successfully.A total of 80 cases were collected,including patients' basic information,symptom and diagnosis,laboratory and radiographic examination result,and the treatment combined traditional Chinese and western medicine.The data were analyzed by SPSS 22.0,and the measurement data with normal distribution used one-way ANOVA test,otherwise used Kruskal-Wallis H test,and the counting data used chi-square test.If the statistical P<0.05,the difference suggested statistically significant.Results1.60 male and 20 female were collected in the subject.The patients'average age is 65.35±14.25,with older than sixty years old in major(68.75%).The underlying diseases of the patients were mainly hypertension(56.25%)and diabetes(36.25%).The admission season was more common in t winter and spring,accounting for 51.25%.Patients had pre-hospital delay time,and the hospital stay was 15.2±10.23 days.2.80 cases had a increasing trend in the laboratory inflammatory index,such as white blood cell,neutrophil percentage,C-reactive protein,procalci-tonin,and the cardiac troponin increased with heart function level.76.25%of 80 cases were ST-segment elevation myocardial infarction,as 23.75%of the Non-ST-segment elevation myocardial Infarction.And the ECG positioning was mainly 36.07%of the anterior and 24.59%of the diaphragmatic surface of heart.Arrhythmia occurred in 30%of patients,with atrial fibrillation or atrial flutter accounting for 58%,which is associated with left atrial enlargement.And LVEF values are related to cardiac function.3.The left heart failure was more common,related to criminal blood vessels were mostly the anterior descending branch of left coronary.Only 55%of the first symptoms of acute heart failure showed typical dyspnea,and the remaining 28.75%showed fatigue,and 22.5%had no symptoms.60%of 80 cases combined with infection.In the cause of the secondary heart failure during hospitalization was over-exertion(40%).4.90%of 80 cases underwent fluid balance monitoring.97.2%cases used urine volume or access volume monitoring,18.06%use central venous pressure monitoring.All cases accepted percutaneous coronary intervention,71.25%rebuild the blood flow with emergency intervention,and the others accepted elective intervention due to intolerance and excessive thrombus overload.Heart failure treatment was diuresis,dilated systemic microcirculation and reconstruction the cycle blood supply when necessary,combined with oxygen therapy,analgesia,sedation,anti-arrhythmia and other comprehensive treatment,which may be related to the deficiency of Qi and Yan of heart.5.In the diagnosis and treatment of traditional Chinese medicine,93.75%of the diagnosis was chest obstruction.The most common type of syndrome was hot blood stasis syndrome,which accounts for 83.75%.The treatment was mainly to promote blood circulation and remove blood stasis,combined clearing heat.The treatment of Chinese patent medicine was mainly based on blood circulation and strengthening the body resistance.Shenfu injection accounted for 57.14%of the most common use.ConclusionAcute heart failure associated with acute myocardial infarction has a high risk of sudden death and mortality,which needs attention and further research.
