Objective: The incidence of coronary heart disease increased year by year with the social development and the better living standards.But the high mortality rate of acute myocardial infarction,which is the acute stage of coronary heart disease,shortens Chinese life expectancy and then brings great burden to the family and society.At this time,the pathogenesis of acute myocardial infarction tends to improve.However,the role of many other related factors on the pathogenesis of acute myocardial infarction are not yet fully understood.The most common pathological process of acute myocardial infarction is that rupture or bleeding of the unstable plaque leads to acute thrombosis formation on the plaque,and then the thrombosis results in acute occlusion of the stenotic coronary,which occurs on the basis of coronary atherosclerosis.The acute occlusion leads to acute myocardial necrosis.Recent related researches found that ischemic necrosis of myocardial cell resulted in local or systemic inflammatory response in the process of occurrence and development of acute myocardial infarction.The two Inflammatory factors-TNF-? and IL-6 are polypeptide cytokines in essence,and they directly involved in the processing of systemic inflammatory reaction,immune reaction and so on.TNF-? is secreted by mononuclear macrophages,then it can start,induce and regulate inflammatory reaction in body.IL-6 is secreted by immunocyte,which immunocyte releases a large amount of at the beginning of systemic reaction,and it can regulate synthesis of C reactive protein in liver.TNF-? and IL-6 play a key role in the start of inflammatory reaction,and to a certain extent,the serum concentration of them represents the degree and stage of inflammatory reaction.Sulfatide is a sulfoglycolipid,and it was named sulfatide in 1884 by Johann Ludwig Wilhelm Thudichum.Sulfatide was known as a major component in the nervous system in subsequent studies.However,recent researches on sulfatide found that sulfatide palys an important role in the physiologic mechanism not only in the nervous system but also in cardiovascular system.The most famous discovery finds that sulfatide has the “dual function” in the blood coagulation system,which means sulfatide has the role of procoagulant or anticoagulation under different conditions.At the same time,other researches reveal that sulfatide involve in the process of atherosclerosis,but it is still not clear that sulfatide is the anti-inflammatory or the pro-inflammatory.Therefore,it is very important to investigate the role of sulfatide in process of coronary heart disease and acute myocardial infarction,which would provide us a new way on prevention and treatment of coronary heart disease and acute myocardial infarction.This study aims to observe the correlation between serum sulfatide and the two inflammatory factors(TNF-? and IL-6)in rabbit model of acute myocardial infarction,and then to validate if sulfatide involve in the process of acute myocardial infarction as a inflammatory factor.Methods: We chosed 6 healthy purebred New Zealand white rabbits,and divided them into A and B groups by random(each group had 3 rabbits).A group was treated as myocardial infarction group and the other as dexamethasone pretreatment myocardial infarction group.We prepared the model by ligation of the rabbit anterior descending coronary artery.We collected venous blood of A group from the ear margin at pre-operation,0.5h,2h,6h,12 h,24h,48 h after-operation.After centrifugation,we extracted the supernatant to measure sulfatide,TNF-? and IL-6.We injected 5mg dexamethasone into B group from the ear margin at pre-operation.And did the same work as A group.Sign of model preparation success is that ECG ST segment elevated,and myocardial pallor or decreased ventricular wall motion in myocardial infarction area was seen.Specimen collection was conducted under a sterile environment with sterile biochemical test tube.Supernatant was centrifugated at 3000 rpm.Serum was stored at-80?.TNF-? and IL-6 were measured by Enzyme Linked Immunosorbent Assay.And sulfatide by matrix-assisted laser desorption ionisation time of flight mass spectrometry(MALDI-TOF MS).Results:1 In A group,TNF-?,IL-6 and sulfatide began to rise at 0.5h aft er-operation,reached peak at 12 h,started to fall at 24 h,and fell back t o normal at 48h;2 In B group,TNF-?,IL-6 and sulfatide began to rise at 0.5h aft er-operation,reached peak at 6h,started to fall at 12 h,and fell back to normal at 48h;3 In A group,we made a pairwise comparison between TNF-?,I L-6 and sulfatide.The results of these pairwise comparison were all pos itive correlation(P<0.05);4 In B group,the pairwise comparison of TNF-?,IL-6 and sulfati de were all positive correlation(P<0.05);5 We made a comparison between A and B group,and found that at the same time locus,the content of TNF-?,IL-6 and sulfatide in A group is higher than B group.Conclusions:1 In group A,the content of TNF-?,IL-6 and sulfatide varied with time,which meaned the content of TNF-?,IL-6 and sulfatide may represent the stage of myocardial infarction in some degree,which also verified TNF-?,IL-6 and sulfatide could reflex the degree of inflammatory reaction in the progress of myocardial infarction.2 In group A and B,the pairwise comparison of TNF-?,IL-6 and sulfatide were all positive correlation,which indicated that sulfatide may be the inflammatory factor in process of acute myocardial infarction.3 At the same time locus,the content of TNF-?,IL-6 and sulfatide in A group is higher than B group,which confirmed sulfaide is a inflammatory factor and it can be inhibited by hormone therapy.Otherwise,the reasonable anti-inflammatory therapy in early stage of acute myocardial infarction can attenuate inflammatory response.At the same time,the peak time inflammatory factors reached in group A is later than group B,which lighten that anti-inflammatory therapy may shorten the course of acute myocardial infarction.
