| Bronchial asthma is a chronic inflammatory disorder of the lungs characterized by reversible spasmodic episodes of dyspnoea,coughing,and wheezing. Many kinds of blood cells and mediators are involved in the pathogenesis of asthma. The pathology of asthma shows the elevated level of eosinophil. Recent studies found that some kinds of inflammatory of bronchial were caused by the infiltration of neutrophil,and there existed a strong association with the occurrence of acute severe asthma,chronic persistent asthma,and asthma inactive to corticosteroid. Bronchial asthma is called asthma disease in traditional Chinese medicine. At present few studies is concerned with the association between TCM syndrome types and microcosmic index. More and more methods are supplied for our further study of asthma with the development of modern medicine and made it possible to define the TCM syndrome types according to the microcosmic index. We employed retrospective methods to analyzed 115 cases of asthma which are recruited from the patients in hospital of Beijing Dong Zhi Men hospital in the last 12 years. Our study includes 66 male patients and 49 female patients,and the average age at onset is 45.5 years old. The ANOVA and chi square test are performed to explored the association between TCM types and microcosmic index through recording the general condition,season at onset,inducement,history,symptom,physical examination,tongue and pulse,blood analysis such as blood cells,serum lipid,lung function test and so on. The statistic software we used is SPSS 11.5. Our data indicates that the age at onset is mainly in the middle. Rates vary by sex and condition of smoking. The ratio of male to female is 1.35:1. The smoker is liable to suffer from asthma. The seizure of asthma is mainly in spring,and is mostly evoked by cold. The study includes 96 cases in phase of acute attack and 19 cases in stable stage. The frequency of Cold-Asthma Type,Heat-Asthma Type and Deficient-Asthma Type in acute attack is 33 person,56 person and 7 person respectively. The frequency of lung-deficiency Type,spleen-deficiency Type and kidney-deficiency Type in stable stage is 10 person,6 person and 3 person respectively. Our study finds that the sum of eosinophil in the patients of Cold-Asthma Type is significantly higher than that of other Types. The sum of leukocyte and neutrophil of Heat-Asthma Type rises up significantly than those of other Types. The level of triglyceride in spleen-deficiency Type increases significantly while the level of haemoglobin is less than other Types of Asthma obviously. The ratio of the practice FEV1 and PEF value in Types of spleen-deficiency and lung-deficiency to the predicted one reaches 80% which is significantly higher than that in other Types,while the ratio in deficient-asthma and kidney-deficiency Type reduces to 40% and is less than Other Types. And the ratio value in Types of Cold-Asthma reaches 60%,and Heat-Asthma is 50%. The sum of basophil,monocyte,lymphocyte, platelet and erythrocyte,and the levels of cholesterol are all no significant difference among Types. Our study indicates that there is a strong correlation between the TCM types of asthma and microcosmic index. Our study also suggests that Cold-Asthma matched with allergic asthma mainly infiltrated by eosinophil,while Heat-Asthma were similar to asthma mainly infiltrated by neutrophil such as intrinsic asthma,acute severe asthma,chronic persistent asthma and asthma inactive to corticosteroid. On the side of management of Chinese Medicine,therapeutic methods of Cold-Asthma is to warm the lung,dispel cold,resolve phlegm and relieve asthma besides adding the anti-allergic drug such as Dilong,Fangfeng,Xinyi,Mahuang,Huangqin,Baihe,Wumei,Yinyanghuo and so on. The therapeutic principle of Heat-Asthma is to clear away heat , resolve phlegm , release lung qi and relieve asthma including the anti-microorganism drugs such as Jinyinhua,Lianqiao,Baibu,Zhuru,Sangbaipi,Huangqin,Huanglian,Shegan,Baijiangcao,Pugongying,Yuxingcao,Banlangen and so on. |
