Advanced Fissure-in-ano is the bursiform or oblong ulcer which is developed by the improper treatment with the acute anal fissure and secondary inflammation. Advanced anal fissure is usually accompanied with indurations in the margins of laceration, anal sinusitis, hypertrophy of anal palilla and external hemorrhoid. In the late period of this disease, the patients suffer much for the severe periodical pain. At present, compared with other methods, operations are the better way to treat this disease although each of them has its advantages and disadvantages. That is why there is no best operative mode in the treatment of advanced anal fissure nowadays. Our research aim of the operation of longitudinal incision and transverse suture is to clear its best clinical indication by analysis the operative procedures in details and postoperative curative observation in order to use this method rightly and timely. For these purposes, 42 cases with advanced anal fissure were subjected to the operation of longitudinal incision and transverse suture and were observed under the technological research requires.METHODS:42 patients with advanced anal fissure were identified in our institution. The application for the treatment of advanced anal fissure is the operation of longitudinal incision and transverse suture. An longitudinal incision is made at the posterior of the anus, accompanied with posterior pectenotomy and cutting off hypertrophy of anal palilla, partly internal sphicterotomy. dividing the top of the incised mucosa and both sides of it, transverse suture the mucosa and sJdn of the incision. The important treatment of post operation is the sufficient antibiotics and stiz bath are given.RESULT: 42 cases are primarily cured, the curative rate was 100%. 3 cases having postoperative incision edema, 1 case not healing on time. The average healing time is 14 + 2 days, no obvious sequels, 3-month follow-up showed no recurrence , anal function was normal.CONCLUSION: This operative treatment has such advantages as good curative effects, less pain, shorter healing time and fewer complications.
