Objective: To investigate the expression of alpha5beta1 integrin and fibronectin in human colorectal carcinoma and determine its clinical significance.Methods: S-P immunohistochemistry was used to examine the expression of alpha5beta1 integrin and fibronectin in the specimens of 58 cases with colorectal carcinoma and 12 cases with colorectal adenoma and 9 cases with normal mucosa. Results: The expression of alpha5beta1 integrin showed significant difference among the normal mucosa, colorectal adenoma and colorectal carcinoma (P<0.05). In different histological specimens of colorectal carcinoma, expression of alpha5beta1 integrin was higher in highly differentiated and lower in poorly, meanwhile, the difference was significant among highly, moderately and poorly differentiated carcinoma (P<0.01). Compared with that in stage Dukes B and C, the expression of alpha5beta1 integrin in stage Dukes A showed no significant difference.But it was higher when compared with that in stage Dukes D (P<0.05). The expression of alpha5beta1 integrin is associated with lymph node metastasis (P<0.01), but not with other clinicopathologic characteristics.The expression of fibronectin showed no significant difference between colorectal adenoma and normal mucosa, but the difference was significant when this two were compared with colorectal carcinoma (P<0.01). In different histological specimens of colorectal carcinoma, there was no significant difference between highly differentiated and moderately differentiated, however, significant difference was observed between the poorly differentiated and them (P<0.01). The expression of fibronectin in stage Dukes A showed no significant difference compared with Dukes B, so did the relation between C and D. But there showed significant difference between the former two and the later two (P<0.05). The expression of fibronectin was associated with lymph node metastasis (P<0.05), but not with other clinicopathologic characteristics.In colorectal carcinoma, the expression of alpha5beta1 integrin was correlated with expression of fibronectin (P<0.05).Conclusion: In colorectal carcinoma, the expression of alpha5beta1 integrin and fibronectin decreased and showed significant difference compared with the colorectal adenoma and normal mucosa. The expression of alpha5beta1 integrin and fibronectin of poorly differentiated carcinomadecreased and showed significant difference compared with the highly differentiated and moderately differentiated. It suggested that the expression of alpha5beta1 integrin and fibronectin might be related to the differentiation of carcinoma and could be regarded as aparameters to evaluate diagnosis of colorectal carcinoma. In Dukes stage, the expression of alpha5beta1 integrin and fibronectin at early stage was higher than that at later stage. Furthermore, the expression of alpha5beta1 integrin and fibronectin was associated with lymph node metastasis but not with other clinicopathologic characteristics. It suggested that the expression of alpha5beta1 integrin and fibronectin might be related to metastasis and invasion of the colorectal carcinoma. Examination of the expression of alpha5beta1 integrin and fibronectin could direct the therapy of colorectal carcinoma. Correlation was existed between the expression of alpha5beta1 integrin and fibronectin. The expression of alpha5beta1 integrin decreased, at the same time, the expression of fibronectin decreased.
