BACKGROUND AND PUnyOSE: Early poststroke depression(PSD) is a frequentand specific entity that impairs the rehabititation and fimctional recovery ofhemiplegic patients. This trial was designed to stUdy the efficacy and tolerance offluoxetine (FLX) and Yu-Lou-shu(YLS) in the treatInent of early PSD. METHODS.Recent hemiplegic patients (<2months)suffering from major depressive desorder(determined by Dragnostic and Statistical Manual of Mental DisordersIV (DSM-IV)and Hamilton's HRDS320) were randomized to assigned to three groups:duringtWelve weeks of basic theraPy, one grouP were beated with fluoxetine (20mg qd),another grouP were treated with YLS (20ml tid),the other were treated only basictreatInent. Patients were evalUaed by use of the HRDS, Zung, Barthel Index (BI),and SNSS. Statistical analysis was performed by using an intent-to-treat approachcomparing the 3 groups at day o (baseline) and weeks 2. 4. 8 and l2 (end point).RESULTS:57 patients were included in the study,19 in the FLX grouP,19 in theYLS group and l8 in the only basic treaAnent (ABT) grouP. There were no...significan defferences in baseline characteristics among the 3 grouPs.The FLX-treated patients camPared with OBT-treated patients demonstrated significanimprovement in mean HRDS, Zung, BI and SNSS at end point (l0.82l6.25[ mean1SD ] VS 18.48 t 6.28,34.15l6.27 VS 40.54l6.25,82.25l9.26 VS74.l2l10.38,49.72l4.07 VS 43.l3 i3.64, respectively, P<0.05).The YLS-treatedpatients compared with OBT-treated patients also demonstrated significant.improvement in mean HRDS' Zung.BI and SNSS at end point (l l .56l5. l9 VS 1 8.48f 6.25,35.24l5.95 Vs 40.54l6.25,87.56l4.84 VS 74.l21l0.38,5l.25 t 3.82 Vs43. l3t3.64 respectively P<0.05). There were no signiflcant differences in HRDS.Zung' BI and SNSS betWeen FLX-treated patients and YLS-treatedpatients(P>0.05).In the meanwthile, there were signiticant defferences in OBT-treated patients betWeen the baseline in mean HRDS \ Zung. BI' SNSS and that of theend point(P<0.05). CONCLUSIONS: Both FLX and YLS is an efficacioustreatment for early PSD. The patient with PSD may recover naturally.
