In this study, the investigation on regulation effect of "Kang shen yi hao" on immune function disorder of senile maintaining hemodialysis patients was performed. "Kang shen yi hao"is a Chinese traditional medicine with function of reinforcing the spleen and the kidney. The investigation is part of the subject under the title of mmune system function of senile hemodialysis and effect of reinforcing the spleen and the kidney The subject is supported by Oongdong Provincial Administration of Chinese Traditional Medicine. Objective: The study was attempted to testify the curative effect of treatment by differentiation of syndromes on immune function disorder of senile maintaining hemodialysis patients. Especially, the curative mechanism was investigated at the level of cytology and molecular biology in order to clarify the pathogeny and disciplines of differentiation of syndromes. The study would also help in improving the "Kang shen yi hao" The major contents of the study are described as follow: Methods :On the designing of experiments: Twenty senile maintaining hemodialysis patients were chosen and divided into experiments and controls randomly. Among the patients, fifteen of them were selected as experiments and other five ones were controls. Levels of cytokins of IL-i P ~IL-2~. IL-6 mRNA in peripheral blood mononuclear cells (PBMC) was compared before and after three months therapy of medicine taking. The levels of cytokines were measured at the end of the first month and the end of the third month to reveal the development of cytokines. On the optimization of experimental conditions: Reverse-transcript polymerase chain reaction (RT-PCR) was applied to detect the levels of cytokines due to its high sensitivity and specificity. The experimental conditions were optimized to ensure reliability of experimental results. The proper conditions were found to be: (1) in the RT stage: the mRNA were incubated at 42 C for 30mm to denature the mRNA- cDNA hybrid: then heated to 99XD for 5mm to inactivate the reverse transcriptase; (2) in the PCR stage: the amplification profile consisted of denaturation at 94 C for 30sec; primer annealing at 58C for 30sec; and extension at 72C for 1.5mm; follow 30 cycles of amplification. Results: 1). Nine of the twenty (45%) patients had mRNA (IL-l13, IL-2, IL-6) expression. The 9 patients were studied. Of the 9 patients with mRNA expression, 6 was in experiments group while 3 was in control group. 2). For the 6 patients in experiments group, levels of IL-2 mRNA increased while levels of IL-i f3 and IL-6 mRNA decreased after three months medicine taking.3). For the 3 patients in control group, only one of them had expression. The level of IL-6 decreased. Conclusion: 1). Only part of patients had interleukin expression, this phenomena was considered to be related to variation of different saniple.2). The comparison of inRNA level of senile maintaining hemodialysis patients before and after treatment indicated that "Kang Shen Yi Hao"improved activity of IL-2 and immunity of T-cells in two ways. First, it increased level of IL-2; second, it decreased levels of IL-I j3 and IL-6. It could be concluded that "Kang Shen Yi ha"had regulation effect on immune function disorder of senile maintaining hemodialysis patients.
