Chronic Coronary Heart Disease With Heart Failure In The Western Drug Treatment Progress

Chronic heart failure is the most common form of heart failure, incidence increasing in recent years, coronary heart disease is the main cause. In recent ten years, there are great progress in the cure and research for chronic heart failure This paper has three parts, mainly discribed the new progress of traditional Chinese and Western medication for chronic heart failure.The first part is about the new progress of modern medication for coronary atherosclerotic heart disease. The guideline of chronic heart failure at home and abroad all described the mechanism, evidence-based medicines and adaptation diseases of basic medicines in great detail, like, angiotension-convert ing enzymes inhibitors, angiotension receptor blockers, diuretics, aldosterone antagonists, beta-blockers, inotropic agents. Recently some scholars proposed some new ideas of cure chronic heart failure by beta-blockers and aldosterone antagonists (eplerenone). Energy metabolic drugs can improve the myocardial ischemia and hypoxia in the energy metabolism and nutritional status, which make they possible to use for chronic heart failure. Since there are more or less side effects to use the traditional medicines, in recent years some new medicines wi th good prospects has been confirmed by clinical trials, like, rennin inhibitor, Ir inhibitor, vasopressin receptor antagonist, diuretic peptide, endothelin receptor antagonist, calcium sensitizer, istaroxime. Some under research medicines, not yet fully support the existing basis for the treatment of heart failure, like, neutral endopeptidase inhibitors, adenosine receptor antagonist, growth hormone, and anti-heart failure medicines in theory or animal studies, like, myosin activator, inflammatory cytokine receptor antagonists, hormone replacement therapy, all opened up new ideas and approaches for treatment of heart failure. Some medicines like statins are not adaption medicine for heart failure, but studies have shown that joint action with other medicines may benefit. This paper discusses the anti-heart failure medicines of its theory, application prospects and the problems exists one by one, and finally makes a brief discussion of the treatment of diastolic heart failure, and the difference with systolic heart failure.The second part is the knowledge of traditional Chinese medicines for chronic heart failure. Chinese medicine has big advantage in treatment for heart failure, the understanding of traditional Chinese medicine is deepening gradually. There are many etiology and pathogenesis discussion, the same point of view is that heart failure is asthenia in origin and sthenia in superficiality, asthenia in origin means deficiency of the qi and yang, sthenia in superficiality means blood stasis, disturbance of water metabolism. Heart failure is normally classified by syndrome differentiation of eight principles and viscera, but differentiation of symptoms and signs for classification of syndrome vary in different reports. This paper classifies it into 6 species according to the literature and experiences of famous doctors. Pharmacology studies and animal experiments of single herb find that Traditional Chinese Medicine to improve the role of hemodynamics in patients, provide a theoretical basis for treating heart failure, then discusses the reports and research of different medicines for heart failure like decoction, injection, capsule, etc.The last part is the expectation of Traditional Chinese'and western medication in treatment of coronary atherosclerotic heart disease. It sums the advantage, shortcomings and related problems need further discussion in both Chinese and Western medication.
