| Background:Chronic glomerulonephritis (CGN) is called chronic nephritis too. Its clinical manifestations appear as a group symptoms with proteinuria, hematuria, hypertension, edema. This disease, with a course of a long slow continuous progress,may have eventually developed to chronic renal failure. It is common but difficult to cure. Research indicates that chronic glomerulonephritis is the first cause of end-stage renal disease (ESRD) in Chinese. To prevent or delay the kidney progressive deterioration and to Improve or alleviate clinical symptoms is the main purpose of the treatment toward chronic nephritis with all the modern technology that medicine makes available. Prevention and cure of severe complications is one of the main purposes. But modern medicine is not good at treatment of chronic nephritis in the early and middle stage. The effect of ACEI and ARB on eliminating the hematuria and urinary protein is embarrassing. The ecurrence rate after discontinuation of drug therapy is much higher and the end-stage renal replacement therapy had brought heavy economic burden to the country and individual. Therefore, seeking a better treatment methods to improve and restore the renal function is currently the main topic about treatment of chronic nephritis. Traditional Chinese medicine therapy for chronic nephritis has shown certain advantages, however the effect of the curative effect proprietary Chinese medicine is not satisfied. The famous and old experts of traditional Chinese medicine have accumulated their rich clinical experience. It is important to sort and verificate their clinical experience and then to be used widely in clinic.Objective:This topic is a part of "11th five-year plan" national science and technology support plan-" The treat safety and effectieness cohort study of LvRenhe professor's treatment for chronic kidney disease based on syndrome differentiation like three pattern types and four syndrome types ". Through the study on LvRenhe's clinical experience and academic thought during the period of the "tenth five-year plan",we have summarized LvRenHe's clinical experience and academic thoughts toward the treatment of chronic nephritis. We confirm the method of yishenbuxu and huayusanjie as the main method of treatment of chronic nephritis.According to LvRenHe's experience of applying medicines,we drawn up to astragalus, yam, glossy privet, GuiBan, danshen, Notoginseng powder, euonymus alatus,semen euryales and fructus rosae laevigatae as the consists of the basic prescription. We have treated 60 cases with the basic prescription to add or subtract according to LvRenHe professor's clinical experience and academic thought. By the way of the clillical observation and study, the effect has been more verified by this method of yishenbuxu and huayusanjie.Method:Theory study and clinical investigation were involved in this study. Selecting and analyzing the knowledge of Chinese medicine on chronic kidney disease. In clinical investigation, the syndrmes differentiated of 60 patients treatead by the method of Yishenbuxuhuayusanjie and the results were observed. The symptonatic scores and main laboratory indexes(GFR,24HUP) were recorded and analyzed after the two months' treatment.Results:After the treatment of yishenbuxu and huayusanjiethe, research result shows that 4 paitents were recoveried,24 people got better markedly,20 ones were improved. Thus the improvement rate was 80%. Compared with before treatment, the glomerular filtration rate was significantly increased (P<0.01); 24 hours urinary protein is obviously reduced (p< 0.01). After treatment,the total score of synptom was significantly reduced (p< 0.01). Compared with before treatment, both the pattern of dual deficiency of yin and yang and the pattern of dual deficiency of qi and yin were obviously improved.Conclusion:Research shows that the method of Yishenbuxu and huayusanjie is an effective method to treat Chronic glomerulonephritis,which have certain effect on improving the clinical symptoms to enhance the quality of life. It is useful to reduce urinary protein to protect the kidney. It is beneficial to improve the glomerular filtration rate of Patients with chronic nephritis to delay their lesions progress. |
