Clinical Observation Of Yinhua Decoction Combined With Minocycline Capsule Treatment Of Acne Vulgaris

Object:In order to explore a kind of economic, secure and equal to promotion treatment of traditional Chinese medicine combined with western medicine for acne vulgaris, observing the effect and security of "yinhua formulation" combined with Minocycline Hydrochloride capsule on degreeⅡandⅢof acne vulgaris (Syndrome Accumulation of heat in the Lung and Stomach). Methods:All of the 97 patients are selected from The Out-patient Department of Dermatology of Traditional Chinese Medicine Hospital which is affiliated to XinJiang Medical University and The NO.1 Hospital of Wuhan (from March,2006 to December,2007). All the patients conform to the diagnostic criteria of acne and are gradedⅡandⅢdegree according to the Pillsbury classification. The syndrome differentiation is Accumulation of heat in the Lung and Stomach, Its skin lesions are red, usually manifested by sebaceous papules, papulovesicle, "Black Head Acne", reddened tongue, yellow or greasy coating, wiry or slippery pulse. All of the 97 patients who aged 18 to 40 and without limitation of sex were randomly assigned into two groups:the number of patients for experiment (Yinhua decoction combined with Minocycline Hydrochloride capsule (brand name:sanrui capsule) (Group A)) And control (Minocycline Hydrochloride capsule (Group B)) groups were 48 and 49 respectively, Both groups applied the ointment (trade name:Youweijia 15g) externally on local area, twice a day。And The Minocycline Hydrochloride capsule were assigned to two groups respectively, the initial dose was 100mg, twice a day, the dose was reduced after two weeks. Group A was added the Yinhua Decoction,200ml each time, twice a day, and taken warmly after breakfast and dinner, The course of treatment are both 6 weeks. According to observing the variation of skin lesions on the 2nd,4th and 6th week of the two groups and calculating the declined counts, the evaluation index of skin lesions as well as the incidence and recurrence rates of the adverse reaction during the treatment, we evaluated the effect and security. Results:(1) After 2,4, and 6 weeks, the effective rate of group A were 45.8%,77.1%,93.8% and that of group B were 30.06%,61.2%,75.5% respectively. And the differences in effective rate of two groups on the 6TH week were statistically significant (P<0.05). Its demonstrated that in contrast to group B which simply used the Minocycline Hydrochloride capsule, the effect of group A which used Yinhua decoction combined with Minocycline Hydrochloride capsule was better in relieving the patients' skin lesions. (2) As the treatment was carried on, the declined counts of skin lesions evaluation of testers in two groups were both keeping rising, after been cured for 8 weeks, compared two groups,, The non-inflammation, inflammation as well as the total declined counts of skin lesions evaluation of experiment group were all distinctively higher than that of control group. Consequently, the differences were statistically significant (P<0.05). (3) After been treated for 6 weeks, observing the secondary skin lesions grade of acne, of experiment group were significantly better than that of control group, and the difference were statistically significant (P<0.05). It revealed that the effect of Yinhua decoction combined with Minocycline Hydrochloride capsule on relieving was better than that of simply using the Minocycline Hydrochloride capsule. (4) Mesuring the serum testosterone (T) level of 49 patients in experiment group. the outcome revealed that the serum T levels of male patients decreased dramatically than before, it was a statistically difference (P< 0.05). while the serum T levels of female patients had little change, so there was no statistically difference (P>0.05). (5) The clinical symptoms of adverse reactions manifested alike to the Tetracycline Drugs in two groups, such as poor appetite, nausea, vomiting, abdominal pain, diarrhea, stomatitis, glossitis, jaundice and so on. The incidence of adverse reactions of experiment and control group were 16.36% and 20.0% respectively, the difference had no statistically significant (P>0.05). (6) After been cured for 12 weeks, we made a trail observation to the well-healed patients for 3 months. The outcome turned out that there were 20 cases healed,4 cases reoccurred and the reoccurrence rate was 20.0% in experiment group, while there were 18 cases healed,10 cases reoccurred, the recurrence rate was 55.6% in the control group. the difference were statistically significance (P<0.05). It demonstrated that the Yinhua decoction combined with Minocycline Hydrochloride capsule could shorten the course of treatment and reduce the adverse effect of antibiotics. Conclusion:Yinhua decoction combined with Minocycline Hydrochloride capsule could improved the skin lesions of acne vulgaris significantly, its clinical effect superior to simple application of Minocycline Hydrochloride capsule, and also could reduce inflammatory rashes, papulovesicle, pigmentations and scars. To some extent, Yinhua decoction combined with Minocycline Hydrochloride capsule could regulate hormone levels;and in the course of treatment, the adverse reaction and recurrence rates of Yinhua decoction combined with Minocycline Hydrochloride capsule were lower than simple application of Minocycline Hydrochloride capsule. Therefore, its provided a way of curing acne vulgaris in combination of traditional Chinese and western medicine, which possessed precise effects, little side effects as well as good compliance to the patients of acne vulgaris.
