Mental Status And Psychotherapy In Perioperative Patients With Cervical Spondylotic Myelopathy

AimTo explore the mental status of the perioperative patients with cervical spondylotic myelopathy and the related factors. To evaluate the differences between comprehensive psychotherapy and simple operative therapy in these patients.MethodsGeneral state of health questionaire, Symptom Checklist 90(SCL-90), Eysenckpersonality Questionnaire(EPQ), Social support rating scale(SSRS) and JOA were used in 100 preoperative patients with cervical spondylotic myelopathy within two days after them hospital admission, Then the patiens were random divided into the psychological intervention group and the control group. The domestic norm were used as control with the result of the Symptom Checklist 90, Eysenck personality Questionnaire and Social support rating scale. The relationship between EPQ, SSRS and SCL-90 were been analyzed. The difference of perioperative SCL-90 and JOA result were used to get the message about the influence after the patients were treated by operation and psychological intervention.Result①The patients with cervical spondylotic myelopathy gained obviously morescores in total scores, total equipartition, somatization, obsession, depression, anxiety, panic and psychosis in SCL-90 than norm (p<0.05) ; the scores of individual sensitivity in SCL-90 were lower than norm (p>0.01) .②The scores of N dimension and P dimension were significantly super than norm in EPQ (p<0.05 );③SCL-90 was positively corrected with the scores of neuroticism. There was also a significant correlation between some factors of SCL-90 and the scores of extrovision and psychoticism in EPQ.④The patients gained more social support than norm (p<0.01).⑤The patients who get more social support scores have better psychologic status than others (p<0.01) .⑥There was no significant difference of the data between two groups before operation; the scores of somatization, depression, anxiety have difference in Intra-sub in the control group(p<0.05); the scores of somatization, depression, anxiety, panic have difference in Intra-sub in the psychological intervention group(p<0.05); the scores of somatization, depression, anxiety, panic have difference in Inter-sub between two groups(p<0.05).⑦There was a significant difference between two groups in the improvement rate after operation(p<0.05).Conclusions①Preoperative patients with cervical spondylotic myelopathy have differentmental disorders such as depression, anxiety and panic.②Personality characteristics, especially neuroticism and psychoticism, are associated with psychological healthy level.③Satisfactory social support is profit for the health of the patients.④Mental intervention is benefit to improve the mental health and functional recovery.
