Backgrounds and ObjectiveAt present,the pneumoconiosis is one of the most risk occupational diseases in our Country,the pneumoconiosis occupies 2/3 of all the occupational diseases according to statistics.The pathogenesis of pneumoconiosis is so complicated that the molecular mechanism of its occurrence and development is unable to be explained completedly,and there is no radical treantment.So the key to control is prevention.It is an important precaution to check susceptibility biomarker and health detrimental effect promptly.Pneumoconiosis is lung fibrosis disease.At present,as domestic and international research data reported,numerous cytokines played an important role in the process of pulmonary fibrosis,transforming growth factor-betal(TGF-β1) was especially paid attention on among numerous cytokines.TGF-β1 can participate the fibrosis course of pneumoconiosis by regulating collagen,FN et al.It can promote the occurrence of pulmonary fibrosis by stimulating extracellular martrix increasing and inhibitting extracellular martrix degradation.Among individual susceptibility factors, great importance was attached to hereditary factors wordwide.At present,some reports had said that the concentrations of TGF-β1 in serum had been correlated with the development of several diseases.Of the known polymorphisms,the T+869C and G+915C in the TGF-β1 gene coding region had been shown to be significantly associated with the serum concentration of TGF-β1.It is necessary to probe whether these gene polymorphisms influence expression of TGF-β1 and susceptibility of pneumoconiosis.The pulmonary functions might reduce with the development of pneumoconiosis and the heart might be endangered with the development of pneumoconiosis. Late-stage pneumoconiosis is prone to produce complication,such as pulmonary heart disease etc.There are no significantly symptoms and signs in the early period of pneumoconiosis,so it is important to observe the change of early health injury in detail according to the recent years' research about mechanism of pneumoconiosis.It is of significance to quest pneumocomosis' early warning,improve the level of health surveillance and strengthen preventive measure.There were 194 coal silicious dust workers(including 90 0+ and 104 dust workers) who were selected in the study.All subjects were all males and Han race. Peripheral vein blood was obtained from all subjects and DNA was extracted from peripheral vein blood.ELISA was used to detect the serum level of TGF-β1. Polymerase chain reaction-restriction fragment length polymorphisms(PCR-RFLP) was used to check TGF-β1 gene polymorphisms in site of+869,+915.Blood lipid kit was used to detect the lipid concentration of serum.The pulmonary functions and ECG of all subjects were examined.The purpose of the research is to probe expression of TGF-β1 and its significence,observe whether TGF-β1 gene polymorphisms in site of +869,+915 influence susceptibility of coal silicosis,and illustrate whether the blood lipid level and pulmonary functions of early coal silicious change.Materials and Methods1.SubjectsThere were 194 coal silicious dust workers(including 90 0+ coal silicosis based GBZ70-2002) who were selected.All subjects were males and Han race with no hypothyroidism,diabetes,pancreatitis,nephrotic syndrome,autoirnmune diseases or other pulmonary diseases.The content of silicon dioxide in coal silicious dust was 5.8%.The range of concentration-short term exposure limit is from 6mg/m3 to 161mg/m3,the geometric mean was 22.96mg/m3;the range of concentration-time weighted average was from 12mg/m3 to 65.38mg/m3,the geometric mean was 27.12mg/m3.The dispersity of dust(particle size less than 2μm) was from 79%to 89%.2.Methods2.1 All subjects were investigated by questionaire including personal data,coal silicious dust contact history,past medical history etc.2.2 5ml peripheral vein blood was obtained from all subjects.2ml peripheral vein blood was used to extract DNA;3ml peripheral vein blood was used to separate serum.2.3 The ELISA reagent kit was used to detect the TGF-β1 content of serum.The operation was strictly executed according to the direction.2.4 Polymerase chain reaction(PCR) and polymerase chain reaction-restriction fragment length polymorphisms(PCR-RFLP) were used to analyze TGF-β1 gene.2.5 Blood lipid kit was used to detect the lipid concentration of serum.2.6 Lung function measuring equipment was used to detect FVC,FEV1. Electrocardiograph was used to detect ECG.2.7 The statistical analysis was performed by using SPSS 12.0.The test level was equal to 0.05.Results1.The mean value of serum TGF-β1 concentration in 0+ group was 49.44±14.43pg/L, and dust workers' was 39.71±13.31pg/L.The mean value of serum TGF-β1 concentration in 0+ group was higher than that in dust workers,there was a statistical diference(P<0.05).Either 0+ group or dust workers,there was no statistical diference between the three genetypes in the concentration of serum TGF-β1(P>0.05);there were also no statistical diferences in different seniority groups,age groups,whether smoking(P>0.05).2.There was the single nucleotide polymorphism in the site of TGF-β1 gene+859, but there were all no statistical diferences between 0+ group and dust workers in distribution frequencies of genotype and allele(P>0.05).There was no single nucleotide polymorphism in the site of TGF-β1 gene +915 in the research,and there was only G/G genotype in the population of the research.3.The content of TC and LDL-C in 0+ group was higher than that in dust workers,and there were all statistical differences(P<0.05);the content of HDL-C in 0+ group was lower than that in dust workers,there was a statistical difference(P<0.05);there was no statistical diference about TG between the two groups(P>0.05).The anormally recall ratios of TG and LDL-C in 0+ group were all higher than that in dust workers,and there were all statistical differences(P<0.05);there were no statistical diferences about TC and HDL-C between the two groups(P>0.05).4.FVC and FEV1.0 value of 0+ group were lower than that of dust workers,there were statistical differences(P<0.05);there was no statistical diference about FEV1.0/FVC between the two groups(P>0.05).FVC index anormal recall ratio of 0+ group was higher than that of dust workers, there was a statistical diference(P<0.05);there were no statistical diferences about FEV1.0 and FEV1.0/FVC between the two groups(P>0.05).There were three workers whose ECG were abnormal in 0+ group;there were four workers whose ECG were abnormal in dust workers.Conclusions1.Transforming growth factorβ1 might participate the occurrence of early coal silicosis.2.The single nucleotide polymorphism in the site of TGF-β1 gene+869 was not related with early coal silicosis,and genetypes had no effect on the concentration of serum TGF-β1;there was no single nucleotide polymorphism in the site of TGF-β1 gene+915 in the population of the research.3.The blood lipid level changed in early preiod of coal silicosis,which might be more likely to result in the occurrence of cardiovascular disease.4.The pulmonary functions reduced in early preiod of coal silicosis;early period pathological changes had no effect on heart function.
