Objective This work was mainly aimed at the following three goals. Firstly, the correlation of changes in serum levels of cytokines with the development and prognosis of lesion was to be investigted among cases with lung cancer compicated with bacteria infection in the primary focus. Then, to be studied was the therapeutic effect of compound herbal formula of Bily Bulb Metal-Securing Decoction (BBMSD) on the pathological process of these cases.Lastly, a research was designed to observe the intervening effects of this therapy on the serum level of cytokines and the association of these intervening effects with progressing of disease for these cases.As a result of these studies, the pathophysiological relationship and its clinical implications between tumorogenesis developing and focus infection were to be explored through such a research among these patients to provide theoretic evidences for programming more ideal therapeutic trial to treat this kind of condition.Methods Fifty-nine patients with lung cancer complicated by bacteria infection in the primary focus were included in this study and they were randomly divided into two groups, i.e.30 cases in treating group (TG) and 29 cases in controlling one (CG). The cases in TG were treated by a combined therapy with routine antibiotics and Modified Bily Bulb Metal-Securing Decoction (MBBMD) and those in CG were treated only by the therapy of routine antibiotics while the therapy against tumor itself was the same for all the cases in the two groups.The serum levels of IL-2, IL-10 and TNF-αand the changes in the sub-population number of T cells were detected among all the cases before and after the treatment. Meanwhile, observed among them were the developing tendency of primary and secondary lesions following the therapeutic intervening and the situation of life quality improving after the treatment.Results There were no significant differences seen in the general clinical data such as the distribution of sex, age, pathological type and clinical staging (P>0.05)between the two groups. Also, no significant differences were found in the levels of serum cytokines, the numbers of sub-population of T cells and the Karnofsky scores of life quality between the two groups before the treatment. After the therapy, the serum levels of TNF-αand IL-10 were all significantly decreased in both groups(P<0.005),while the difference of changes was more obvious in TG than in CG before and after the treatment(P<0.01).Furthermore, the serum level of IL-2 and the ratio of CD4/CD8 were markedly elevated in TG following the treatment (P<0.005),with a very significant difference between TG and CG in these indicators (P<0.01).In contrast, the increase of these two indexes was not significant in CG.(P>0.05).The total effective rate of improvement in patient's Karnofsky scores was 96.67% (improved rate plus stable one), significantly superior to that of controls (72.41%, P<0.05).Conclusions The anti-inflammatory treatment was very effective for the treatment of complicated infection in the focus of primary malignancy in both groups of cases.However, the combined therapy with routine antibiotics and MBBMD was more effective than the one only with routine antibiotics.The combined therapy could significantly improve life quality, reduce the serum TNF-αand IL-10 levels, and elevate the serum IL-2 level and CD4/CD8 ratio among all the cases in treating group to make the general conditions of these patients improved markedly. These results suggested that MBBMD should be beneficial to the combined treatment for patients of lung cancer complicated with bacteria infection in the primary focus to reach more ideal therapeutic effects.These effects might be the results of intervening with the pathophysiological process of such a kind of lesion by BBMSD and be brought about via strengthening the immunity and inhibiting the inflammatory reaction in this case.
