Purpose: To investigate the traditional Chinese medicine(TCM) pathological mechanism of diabetic peripheral neuropathy(DPN) through the aspect of qi deficiency and blood stasis, so as to provide an important theoretical basis for the treatment of DPN with the method of replenishing qi and activating blood.method:The literatures which were related to the etiology, pathogenesis, qi deficiency and blood stasis, method of replenishing qi and activating blood and DPN were searched from the ancient and modern medical books as well as all kinds of periodical databases. The relation between qi deficiency and blood stasis, method of replenishing qi and activating blood as well as the disease was investigated through the most pathological mechanism of DPN, close correlation between TCM qi deficiency and blood stasis syndrome as well as DPN. The effects of replenishing qi and activating blood method in clinical practice and animal experiments by the traditional Chinese medicine experts were summarized, in order to verify the reliability and validity of replenishing qi and activating blood method in the treatment of DPN.Results:The state of an illness is recurrent in DPN patients and the treatment time is longer, which brings a great physical and psychological suffering to patients. Through referring to all kinds of ancient and modern literatures, the pathogenic factors of DPN were closely associated with Qi and blood. Qi deficiency and blood stasis became the most critical pathogenic factors. The pathological feature was root deficiency and branch excess, mostly qi deficiency. The blood stasis became a key pathological link; The pathological mechanism of qi deficiency and blood stasis in TCM was similar with DPN microangiopathy. The method of replenishing qi and activating blood could significantly reduce the numbness, pain, cold limbs and other prominent symptoms of DPN, accelerate the patient’s blood circulation system, improve the nerve conduction velocity, effectively inhibit platelet accumulation, so as to gradually strictly control the disease progression and deterioration.Conclusion:1.Qi deficiency and blood stasis are the pathogeneses of DPN. 2.The method of replenishing qi and activating blood becomes the most effective method in the treatment of DPN.
