Rheumatoid arthritis is a kind of autoimmune disease which mainly encroach on symmetrical multi-joints. Joint pain, swelling, function limited are the main clinical manifestations of RA, which with high teratogenicity and disability rate, seriously affect the patients’quality of life. So it is significant to investigate RA’s impact on patients QoL and explore the effection of chronic disease management’s intervention.The first part is a literature review, which discusses the concept, current situation, influencing factors and traditional Chinese medicine cognition of the QoL of patients with RA, and the current situation, TCM cognition and using in RA of the chronic disease management.The second part, containing two chapters, focuses on clinical research:I. A survey of RAQoL.Objective:Use SF-36, SAS and SDS scale to evaluate the QoL, anxiety and depression condition of RA patients and healthy people, then analyze the possible influencing factors about it.Methods:The SF-36, SAS, SDS scale were used to investigate the RA group and the healthy adult group, each group includes 40 cases. The results were examined, and the possible influencing factors were analyzed.Results:Compared with the healthy adult group, RA patients’ score in the 8 dimensions of SF-36 scale and the PCS、MCS were lower, while the SAS and SDS scale score is higher than that of healthy control group (P<0.05).Gender, education degrees, occupations, marital status, incomes, types of medical expenses and living area have varying degrees of influence in RAQoL (P<0.05).Conclusion:RA patients’ QoL level is lower than the general healthy people, which relates to the physiological, psychological, and multiple aspects. They are more likely to suffer from anxiety, depression and other negative psychological state. Gender, education degrees, occupations, marital status, incomes, types of medical expenses and living areas are related to it.Ⅱ. Effects of chronic disease management on disease control and QoL in patients with RAObjective:Through intervening RA patients with management measures of chronic disease with TCM features, to observe its effects on patients’disease control and QoL, and to investigate the effection of chronic disease management on disease control and QoL of RA patients.Methods:Through non randomized concurrent controlled trial design,70 cases of RA patients who meet the inclusion criteria were divided into the treatment group and the control group,35 cases in each group. The treatment group was given chronic disease management measures on the basis of the routine drug therapy, while the control group was only treated with conventional drugs. The treatment course is 12 weeks. The disease control level such as VAS, DAS28, ESR, CRP, and the average synovium thickness, and the QoL of patients such as HAQ, SAS, SDS of the two groups were observed before and after the intervention.Results:Compared with the control group, patients of the treatment group with chronic disease management intervention have lower VAS, DAS28, CRP and average synovium thickness level(P<0.05). SAS, SDS scale score are also lower than the control group, while the HAQ score difference is insignificant (P>0.05).Conclusion:The interventions of chronic disease management in RA patients are benefit to the disease control of patients’ condition, it can also improve the anxiety and depression caused by RA, improve the quality of life in RA patients.
