| Today,"joining the WTO"has became the most popularly used word in China undoubtedly. Every industry is inevitably inpacted by internationalization.The medical treatment is the one of markets which the Chinese government has opened to the outside world. Although the present situation the public still cannot feel the obvious changes, the hospital president has been dear in his mind that aches would be coming. How to improve the quality of hospital service and to ascribe the patients as the center is the problems to which medical treatment faces up to the competition of the foreign-invested medical relating necessarily. But that the medical informationization's level being high or low is the important quality index which evaluates the hospital's comprehensive strength. In the developed country it has been an important symbol of evaluating whether or not a hospital has good image and advancement whether it possesses a hospital information system of integral function or not. It is the crux of the matter that can raises the management level of existing hospital and explores the system by advanced the software technology in devising and exploring a HIS system which has a high scalability. The paper is backed with the project of 999 hospital of ShenZhen, and takes clinic register's system as an example to expound. After introducing the current situation of the development of the information system of the hospital and the two layers strctrue and three layers structure,the paper indicates that to use Windows DNA system structure, and adopt program design theory based on COM to guide system development can set up application system having high scalability, thus solve all sorts of technologic difficulties that existing HIS system faced with.Windows DNA is the strategic structure for the distributed application based on component of Microsoft. It is based on COM component and combined COM + services to implement the distributing operating environment. Three layers structure of Windows DNA are generally divided into user's interface layer, business logic layer and data stored layer. Each layer is relatively independently, and communication between layers is through public interface. The loose coupling between layers can simplify program design, and improve scalability of program.Three layers structure of Windows DNA is more rational, has more scalability than C/S/ and B/S structure.The paper has studied and probed into the Windows DNA, and had set up clinic register subsystem based on Windows. As the core of Windows DNA, COM and COM + technology conglutinate and link up DNA's each layer. COM is a model designed by Microsoft adopting component design method.It defines communication standard between components.The design method based on component has inherited and developed object-oriented programming design method (OOP). It applies object technology to system design and abstracts OOP realization process further.The design method based on component emphasizes the real software reuse and strong cooperation. Using the design method of component based on COM, we can design and realize the component having good reuse and extension,and lay a solid foundation for setting up an application having high scalability. COM + is developed on the basis of COM, includes the merits of DCOM and MTS, and offers more system services and good solution for developer on how to set up an application with high scalability. After mastering Windows DNA system structure based on COM+ technology,we can use it to design HIS system having good scalability. When analyzing main business logic and workflow of clinic register, functions needing to finish in system are summed up.
