Picture archiving and communications system (PACS) is a very important component of hospital information system, and is the application of information technology to hospital, and is the key link of hospitals' digitalization. DICOM is an international standard of storage and communications of medical image. The ministry of health requires Chinese hospital to follow this standard. PACS based on DICOM is the current, but the study of DICOM is underway in China, and there is no PACS according with DICOM, and foreign's PACS is expensive and not answer for Chinese doctors' customs. In this case, studying storage and communications of medical image according with DICOM is very significance.In the first place, roundly introducing PACS, including the constitution of PACS,topology structure,design principle,problems to be attentioned,evaluating benefit and the standards. Then expatiated DICOM, including its history,systematic structure,characteristics,application and information model. Based on those, instancing a hospital's PACS, designed communications and storage of medical images modules according with DICOM in PACS, coperated with Great Wall System Co., Lmt, Nanjing. Based on the modules, developed DICOM file decoding,media storage,communication,film printing and DICOM gateway using visual environment under Windows, and gave detailed implementation flow and the results. During implementing decoding of DICOM files, found three file formats different from the standard format, and decoded the three kinds of files successfully. During implementing DICOM media storage, based on the five basic operations, extended two operations, so extended the functions of DICOM media storage; From a new point –data flow designed the moduls. In implementing DICOM communications, fulfilled C-STORE, C-MOVE and C-GET etc, and deal with the abnormities. In film printing, DICOM images could be printed as clinical needs on DICOM printers through standard interface, and could be printed in many formats. In DICOM gateway, implemented the conversion between DICOM images and normal images (BMP and JPEG). The modules is being applying in PACS, brings benefits to hospitals.
