Health human resources is an important factor to promote sustainable,Stable development in health service, and a decisive source for improving the soundness of health care system, raising the quality of service and promoting the health care endeavor. The quantity, quality and professional structure and allocation of health human resources have an impact on the ability of health service provision and dictate the quality of medical care. To achieve common access to basic health services, to reduce gap between the urban and rural residents'right to basic public health services, we must build a well structured, reasonably deployed and high-quality health-service force who can meet the requirements in the development of socialist market economy and health service.Purposes: To understand the status of health human resources in primary medical care institutions in Jilin province, to evaluate regional differences in primary medical care systems, to provide basic data for reasonable deployment of human resources in the health care system, and to provide suggestions on problems existing in the allocation of human resources for medical service.Methods: An Excel-supported database was established to aggregate the survey results. SPSS13.0 statistics tool was employed to analyze the data.Results:1. 113 primary medical care institutions were investigated; total number of medical staff surveyed was 2 309; there is a population of 2 203 019 in the jurisdiction covered by the service of these institutions.2. Different medical institutions differ in the gender composition of the health workforce. In the community health service centers, township hospitals and community health service stations, the proportions of men and women in the medical staff are 0.47/1, 0.98/1 and 0.30/1, respectively, with women more than men; while in the village clinics, the number of men is more than that of women, with a ratio of 2.91/1.3. The supply of health workforce in the primary medical institutions is insufficient, and there is1.05 health care personnel (rural doctors included) per thousand residents; the quantity of health care personnel (rural doctors included) in urban and rural medical institutions of the jurisdiction covered are 0.76 and 1.52 per thousand residents,respectively.4. The overall education level for health human resources are relatively low, with college and higher education institute graduated staff accounting for only 38.55%.There is a considerable gap between urban and rural distribution of human resources for medical care, for example, in the urban primary medical institutions, graduates from college and Higher Educational Institutes and graduates from secondary technical schools account for 50.71% and 43.48%, respectively; while in the rural primary medical institutions, the above rates are 26.66% and 61.67% respectively.5.Certified doctors and registered nurses are lacking in the primary medical institution is insufficient,the number standing at 0.35 certified doctors and 0.21 registered nurses per thousand residents.This falls far short of the required level set in "Health and Human Development in China 2001-2015 Program".6. Imbalanced ratio of doctors to nurses. The doctor-nurse ratio in primary medical institutions is 1.68/1, with nurses lacking. The doctor-nurse ratio in urban and rural medical institutions are 1.63/1 and 1.75/1 respectively; lower than the required rural ratio 1/1 and urban ratio 1/2 as required by the Ministry of Health.Conclusions: Health human resources in the primary medical institutions in Jilin province are obviously short in supply and the education level for the human resources has yet to be improved. There is a regional imbalance in health human resources in terms of staff number, professional structure and deployment. The doctor-nurse ratio is less than reasonable, and the number of nurses is inadequate.In response.we should further strengthen the management the allocation of health human resources, improve their overall educational level, elevate the professional entry criteria, provide regular on-the-job trainings to improve the quality of existing medical workers and make preferred policies to encourage the medical and technical personnel to serve in primary medical institutions. In addition we should adjust the structure of health human resources so as to improve the comprehensive strength of primary health care and medical service institutions in Jilin province.
