Objective To evaluate the visual quality of myopia of middle-aged patients after laser in situ keratomileusis monovision. Methods 100 myopic patients were aged from 35 to 45 years.Corrected vision were all over 1.0 in myopia.Spherical equivalent ranged from -2.00 to -12.00D. The patients were divided into 2 groups .In group A, 50 myopic patients were treated by laser in situ keratomileusis(full distance correction) as control. In group B ,50 myopic patients were treated by laser in situ keratomileusis monovision (dominant eye corrected for distance vision and the nondominant eye for near vision). The changs of distance vision, near vision ,contrast sensitivity and stereopsis vision acuity were measured wearing glasses before surgery and at half-month, 1 month,3 months after surgery; Amplitude of accommodation was measured before surgery and after surgery. At 3 months after surgery self-perceived quality of vision and satisfaction were investigated. Results (1)Between A and B had no significant difference of postoperative binocular uncorrect distance vision( P > 0.05),but significant difference of postoperative near vision of naked eyes(P<0.05).(2)Patients'contrast sensitivity descended early after half month and restored to their preoperative level of wearing glasses at 3 monoths after surgery. There was no significant difference about the contrast sensitivity of postoperative half-month ,1 months and 3 months between A and B ( P>0.05).(3)Stereopsis vision acuity descended at half month after surgery, increased gradually at 1 month ,3 months after surgery, but still decreased(P>0.05 ) .There was no significant difference of stereopsis vision acuity of postoperative half month ,1 months and 3 months between A and B(P>0.05). (4)At half month postoperatively, amplitude of accommodation of dominant eyes and nondominant eyes was significantly smaller than that before surgery in group B( P<0.05).There are increased gradually at 1 month ,3 months after surgery, but still decreased(P>0.05).(5)All patients had no dysmorphopsia, chang of color vision ordistortion after surgery. 22.7% of patients required reading glasses in group A,but monovision patients never needed. There was no significant difference between A and B of satisfaction of distance vision at postoperative 3 months, but significant difference of near vision. Conclusion LASIK monovision correction represents a viable method of correcting middle-aged myopia patients considering refractive surgery, was better than LASIK.It improved morely quality of vision.
