Clinical Study On Patients With Mid And Late Stage Gastric Carcinoma Treated By Invigorating Spleen And Warming Yang Combined With DCF Chemotherapy

Objeetive: In basic of the chemotherapy treatment, with the main line of invigorating spleen and warming yang", observe the effect of the invigorate spleen and warm yang decoction in the treatment of advanced gastric cancer and study the mechanism.Methods:According to the diagnostic criteria of western medicine and TCM, assessment criteria of clinical effect, and in the light of the relevant standards refer to the China's cancer diagnosis and treatment of common norms and guiding principles of Chinese medicine clinical research new drugs(PRC Ministry of Health, Medical Administration). Selected 40 patients with advanced gastric cancer were randomly divided into treatment and control groups.20 patients in treatment group received 2 cycles of chemotherapy until the end, continuing to take invigorate spleen and warm yang decoction and joint chemotherapy, the specific drug is:TXT 35-50mg/m2, ivgtt 1h, dl,8,5-FU 500-750 mg/m2 ivgtt, dl-5, DDP 20mg/m2ivgtt, dl-3,, intravenous infusion. every 3 weeks as a cycle,,2 consecutive chemotherapy cycles.20 cases in control group, simply to DCF chemotherapy. After 2 cycles, patients were compared changes in clinical symptoms, living conditions, tumor markers and the degree of chemotherapy side effects, and statistical analysis of differences between two groups.Results:This study showed that the clinical symptom score after treatment, the difference was not significant (P>0.05), balanced than; clinical symptom score after treatment, the difference was statistically significant (P<0.05).TCM treatment group,6 cases were markedly effective, effective in 13 cases,1 patient; the control group that were two cases of 8 patients were ineffective in 10 cases.The t-test, the difference was significant, indicating that TCM treatment group more effective than the control group.Improve bloating symptoms in the treatment group compared with control group, there was significantly (P=0.025<0.05); in improving the symptoms of the stomach compared to the control group, the difference was statistically significant (P= 0.042<0.05);belching symptom improvement compared to the control group, the difference was statistically significant (P=0.042＜0.05); in improving the symptoms of the less satisfied than the control group, the difference was intended (P=0.042<0.05); in fatigue symptomsimprovement, compared the two groups showed no significant (P>0.05).Two groups before treatment QOL score by comparison, the difference was not statistically significant (P> 0.05), balanced than.QOL score after treatment, the difference was statistically significant (t= 3.022, P= 0.004<0.05).KPS score after treatment to enhance the observation group and 8 cases, stable in 11 cases, lower in 1 case, the control group were 3,13,4 cases, the two groups by rank sum test, the difference was statistically significant (P<0.05).Weight gain after treatment observation group,6 cases, stability in 11 patients, decreased in 3 patients in the control group were 3,7,10 cases, the two groups by rank sum test, the difference was statistically significant (P <0.05), whilegroups in the CEA, CA199 value, toxicity and side comparison, the difference was not statistically significant.Conclusion:Using invigorate spleen and warm yang decoction combined with chemotherapy than chemotherapy alone in treatment of advanced gastric cancer, in improving clinical symptoms, the quality of life and weight, the treatment group is better than the control group;.But have no effect in CEA,CA199 level, and did not increase the toxicity. Invigorate spleen and warm yang decoction is the effective way in advanced gastric cancer.
