Objective To introduce the experiences of total hip replacement (THR) in the treatment for dysplasia of the acetabulum in adults and follow up the cases that has been treated.Methods From Mar 2002to Mar 2009,47 hips in 46 adult patients with acetabular dysplasia undergone total hip replacement were included in this study. The number of males and females was 14 and 32 respectively. The age of the patients at surgery ranged from 20~76 years (mean 48.8 years).45 patients had a unilateral total hip replacement and one patient had a bilateral total hip replacement.Result The incision healed well, and the follow-up time ranged from 6 months to 83 months (mean 32 months). All patients could walk independently, take care of themselves and return to previous job in 6 months postoperatively. The average Harris score was increased from preoperative 51.83 to 85.93 points in one year after operation. So far, there has been no revision.Conclusion THR is an effective method in treating osteoarthritis secondary to dysplasia of the acetabulum in adults and a challenging work to the Orthopaedic surgeon. It is critical to understand the complexities of total hip replacement. The careful preoperative planning should be made. The technical difficulties encountered during operation included:correction of the length of bilateral lower extremities, balance of the abductor muscles, release of the soft tissue, dealing of the superior segmental defect of acetabulum and selection of the right components.
