Objective To explore the correlative factors for recurrence of ovarian endometriosis after conservative surgery and to illustrate the mechanism of Traditional Chinese Medicine to keep ovarian endometriosis from recurrence.Methods A retrospective analysis was performed on 96 patients who had underwent conservative operation to diagnose ovarian endometriosis.The factors measured included age at surgery, gravidity,parity,the serum level of CA125 before operation,type of operation(laparoscopy or laparotomy), intraoperative r-AFS, postoperative medication, the recurrent numbers and so on. There were 40 patients who were devided into two groups,treated by Chinese medicine and western medicine respectively. They underwent clinical interview,physical examination,serologic examination and ultrasonography. All dates were analyzed by SPSS software.Results ONE:1.The average onset age was (31.46±5.64) years old.96 patients with OEM after conservative surgery were followed-up in 5 years and 15 cases recurred.The accumulated rate was 15.6%(15/96),among which, the western medicine group was 23%(6/26),the Chinese medicine group was 14.7%(5/34),the untreated group was 11.1%(4/36). Nine cases recurred in the first year after conservative surgery.2.There was no correlative relationship between recurrence and gravidity,parity, sides of ovarian cysts, serum level of CA125 before operation.3.There was significant difference in the average medication time, (4.35±1.37) months of the western medicine group and (6.82±3.49) months of the Chinese medicine group, respectively.TWO:1.There were 85% patients in the third or fourth surgical stage and the average of r-AFS score was (41.45±21.71) in the Chinese medicine group, but 95% patients in the third or fourth surgical stage and the average of r-AFS score was (69.20±25.78) in the western medicine group, P<0.05.2.The symptoms score was (9.20±5.60),(2.06±1.60) and the signs score was (1.40±1.14),(0.24±0.75) in the Chinese medicine group before and after treatment, P<0.05. The symptoms score was (5.30±3.39),(11.68±1.20) and the signs score was (2.50±1.96), (0.16±0.50) in the western medicine group before and after treatment, P>0.05.3.During the first year after their conservative surgery,the recurrence rate was 10%,treated by traditional Chinese medicine and 5%,treated by hormone drugs.The pregnancy rate was 15%,5%, respectively.Conclusion The surgical stage, postoperative medication and medication time are the most obvious risk factors for recurrence of ovarian endometriosis after conservative surgery,but not those as follows:gravidity,parity, the sides of ovarian cysts, the serum level of CA125 before operation. The highest recurrence rate happens in the first year after conservative surgery. The recurrence rate would be increased if doing longer follow-up. The patients who were in the third or fourth surgical stage or with higher r-AFS have an tendency to choose postoperative medication.The traditional Chinese medicine with the function of invigorating the kidney and removing blood stasis can improve symptoms,pregnant rate and delay recurrence.
