Conservative Surgery For Ovarian Endometriosis Patients With Pregnancy Outcomes Impact Analysis

Objective:To retrospective study on pregnancy outcomes of whom underwent conservative surgery with ovarian-endometrioma,and analysis of pregnancy-related factors.To probe methods that improve fertility chance after surgery.Method:There are 454 patients undergoing surgery treatment because of ovarian endometriosis cyst in Tianjin central of obstetrics and gynecology hospital from May 2012 to May 2014,whose postoperative diagnosis confirmed by pathology.257 patients had requirement of fertility,who were followed by telephone after surgery,and data compared after follow-up 48 months,47 cases are lost,210 cases of patient clinical data are effective collected.(1)They have been divided into two groups in terms of pregnancy outcome: 134 women after surgery had a successful pregnancy outcome during follow-up period,76 cases were still to fail to get pregnancy at end of following time(3 cases were unmarried among them);COX univariate analysis shows whether the outcome of pregnancy were related with age,cyst size,r-AFS stage and so on;(2)All cases were grouped into natural pregnancy and ART using by means of interference after operational manegment.Results:1.127 cases were with successful pregnancy outcome after surgery in 210 patients,total accumulated pregnancy rate for 65.2%,which medication group and non-medication group patients cumulative pregnancy rate is respectively 54.4%(68/125)and 72%(59/82),did not show any statistical significance between two groups(p>0.05);Meanwhile each periods had significantly improved in pregnancy rate,I-II,and III-IV stage patients cumulative pregnancy rate respectively for 67.5%(54/80),and 57.5%(73/127).There were no significantly differences in pregnant outcome among each periods(p>0.05).2.There were no significantly differences in median time of pregnancy betweenthe groups(p>0.05).In each group,the peak of pregnant rate occured in 4-months after operation,accumulated pregnancy rate 22.8% is higher than other period,16.5% cumulative pregnancy rate after 5 months,then gradually declined.But 1.5 years later still has a successful pregnancy chance after operation.3.Successful pregnancy cases were divided into natural pregnancy and assisted reproductive fertility group.There were to segment patients aged...
