Part I.Clinical Research:Objective:To evaluate the observation of curative effects of the traditional Chinese medicine (Fufangtongluo beverage)on the patients with cerebral infarction at convalescence.Methods:Thirty patients with cerebral infarction at convalescence of the Nanjing hospital of Traditional Chinese Medicine from August 2009 to March 2010 in line with Chinese medicine and Western medicine diagnosis standards are brought in the research.The patients are randomly divided in Fufangtongluo beverage of the treatment group and the control group, treatment group of 17 cases and the control group of 13 cases, two groups obtained norms of standard western medicine treatment programmes include:antiplatelet aggregation,regulating lipid, antihyper-tensive,reducing blood sugar and so on. In addition to the treatment group plus the medicine of Fufangtongluo beverage every 100 ml, twice a day;all patients for four weeks treatment, during treatment except other Chinese medicine of removing blood stasis and nourishing the liver and kidney.Observation patients with the integration of TCM Syndrome and National Institute of Health Stroke Scale(NIHSS),von Willebrand factor,thrombomodulin, endothelin, matrix metalloproteinases-2, matrix metalloproteinases-9 in blood plasma. All records of the research are given statistics analyze.3.Results:(1) The results showed that effects of Fufangtongluo beverage on patients of cerebral infarction at convalescence with the integration of TCM Syndrome and NIHSS, both the treatment group is more superior, with statistical significance as compared with the control group(P<0.05).(2) The results showed that effects of Fufangtongluo beverage on the index change of vWF,TM,ET the treatment group is more superior, with statistical significance as compared with the control group(P<0.05).But there was no statistical significance on the index change of MMP-2,MMP-9(P> 0.05). 4. Conclusion:With standard western medicine treatment on the basis of standardized, Fufangtongluo beverage can improve of the integration of TCM Syndrome and NIHSS and vascular endothelium cells function of patients with cerebral infarction at convalescence,and some research results better than Western medicine treatment, suggesting Fufangtongluo beverage with a degree protective effects on vascular endothelium cell function and improving symptoms.Partâ…¡. Experimental study:1.Objective:To observe the prevention protect on cerebral and mechanism of Fufangtongluo beverage on the model rat of middle cerebral artery ischemia-reperfusion (MCAO) and human umbilical vein endothelial cells (HUVECs) injury caused by hypoxia-reoxygenation.2.Methods:The MCAO rats are fed one week with Fufangtongluo beverage by gavage before operated.We observe indicators such as the neurological behavior score, the rate of cerebral infarction, brain index and brain water content and brain tissue content of Evans blue to establish whether Fufangtongluo beverage has protective effect on the MCAO rats. We observe von Willebrand factor (vWF), thrombomodulin (TM) content and matrix metalloproteinase-2 (MMP-2)mRNA expression in the plasma and the brain tissue nitric oxide synthase (NOS), superoxide dismutase (SOD) activity and malondialdehyde (MDA) content as indicators to observe the mechanism of Fufangtongluo beverage effect on the MCAO rats endothelial cells injury. HUVECs are cultured in vitro to build the hypoxic-reoxygenation model, then we observe cell morphology by inverted microscope,and determinate cell viability by MTT, and detect lactate dehydrogenase (LDH), endothelin (ET), nitric oxide (NO), nitric oxide synthase (NOS) level in cell culture medium to observe the mechanism of Fufangtongluo beverage protective effect on human umbilical vein endothelial cells (HUVECs) injury.3.Results:The brain preventively protective effect on (MCAO)rats:In the group of MCAO rats, Fufangtongluo beverage can significantly reduce neurological behavior score, the rate of cerebral infarction and brain tissue content of Evans blue, compared with the model group (P<0.05);can significantly reduce plasma vWF,TM content with model rats, compared with the model group (P<0.05), and inhibit MMP-2 mRNA expression; can significantly reduce MDA content,increase SOD activity, compared with the model group (P<0.05, P<0.01), and reduce NOS activity. The mechanism of protective effect on human umbilical vein endothelial cells (HUVECs) injury: Fufangtongluo beverage can significantly inhibit cell morphology change and endothelial cell injury caused by hypoxia-reoxygenation and effectively reduce LDH release(P<0.05,P<0.01); Fufangtongluo beverage can inhibit the hypoxia-reoxygenation-induced ET release, improve NOS activity and promote NO release (P<0.05, P<0.01).4.Conclusion:Fufangtongluo beverage has the preventively protective effect on (MCAO)rat's brain. The mechanism maybe it can reduce plasma vWF, TM content, inhibit expression of MMP-2 mRNA;it can reduce NOS activity and MDA content and increase SOD activity, which has protective effect on blood vessel endothelial cells. Fufangtongluo beverage has obviously protective effect on human umbilical vein endothelial cells injury. The mechanism maybe it can inhibiting the release of ET,enhancing NOS activity and promoting the release of NO to relieve the human umbilical vein endothelial cells (HUVECs) injury caused by hypoxia-reoxygenation.
