The Clinical Analysis Of Distributing Characteristics Of The Traditional Chinese Medicine Syndrome Of Acute Gouty Arthritis

Objective:Analyze and summarize the distributing characteristics of the traditional Chinese medicine(TCM) Syndrome of acute gouty arthritis, provide a certain basis for standardized research of acute gouty arthritis'TCM Syndrome.Methods:Sum up the distributing characteristics of acute gouty arthritis'TCM Syndrome many physicians think in recent years and ancient knowledge of the disease, propose that the clinical syndrome of acute gouty arthritis should divided into:Blockage of damp-heat as the based syndrome, clip stasis or clip wind or clip phlegm based on the first one. Collected 100 cases met the diagnostic criteria for acute gouty arthritis cases, according to a unified standard TCM syndrome differentiation, statistics separately the age of onset, the course of disease, clinical manifestation, signs, tongue, pulse, incentives and major laboratory indicators(uric acid, erythrocyte sedimentation rate, C-reactive protein, blood pressure, blood sugar, blood fat) and other relevant data, analyze objectively the distributing characteristics of acute gouty arthritis' TCM Syndrome.1.Blockage of damp-heat:accounts for 38% of all cases, the phenomenons of joint pain, burning, swelling are more prominent, and the proportion of the patients with metabolic syndrome related diseases and incentives is higher than the other three.2. Blockage of damp-heat folder stasis:accounts for 21% of all cases,in addition to joint pain, burning, swelling,other symptoms can be seen,such as ecchymosis, scleroma, duration is longer than the other three,body mass index, red blood cell (RBC) higher than the blockage of damp-heat folder phlegm, the blood uric acid level higher than the blockage of damp-heat.3.Blockage of damp-heat folder phlegm folder wind:accounts for 20% of all cases, in addition to joint pain, burning, swelling, the symptom that the pathological joints are not fixed is its characteristic, simpler than the other three, erythrocyte sedimentation rate(ESR) higher than the blockage of damp-heat4.Blockage of damp-heat folder phlegm:accounts for 28% of all cases, clinical manifestation is complex, duration is longer than the blockage of damp-heat and the blockage of damp-heat folder phlegm folder wind,cases with metabolic syndrome-related diseases are relatively more common, laboratory tests have no special features.Conclusion:The distributing characteristics of acute gouty arthritis'TCM Syndrome:wet, hot is the pathological basis; stasis is an important pathological factors; exogenous pathogenic wind is a predisposing factor; phlegm is an important pathological characteristics, moisture, heat, blood stasis, wind, phlegm are the main syndrome elements.The blockage of damp-heat alone is the most common syndrome, followed by the blockage "of damp-heat folder phlegm, the blockage of damp-heat folder stasis, the blockage of damp-heat folder wind also have a certain percentage. AS the characteristics of the acute gouty arthritis syndromes are complicated,by extensive literature research and many years'experience of instructor, we propose five syndrome elements of wet, heat, blood stasis, wind, phlegm. Next through the application of Logistic regression analysis,the most relevant syndrome factors can be obtained, to simplify the syndrome, to provide further objective evidence for TCM syndrome differentiation. As the sample size is limited, each syndrome has its own characteristics,If this results can represent the characteristics of the general acute gouty arthritis syndromes, we need take a step forward.
