The TCM Syndrome Analysis Of Gouty Arthritis

ObjectiveIn recent years, with the development of Chinese economy and the improvement of living standards,the prevalence of gouty arthritis is increasing.lt has been one of the common metabolic disease in today’s society. The treatment of gouty arthritis in modern medicine are pain relief and symptomatic treatment. So more and more research shows that traditional Chinese medicine in treating gouty arthritis has a remarkable effect. The treatment price is lower than modern medicine, while less side effects, and higher security. But nowadays, the unified specification standards of syndrome differentiation as where as the treatment in gouty arthritis is still blank.PurposeThis study collected the related dates of gouty arthritis patients through the retrospective investigation, and the mainly dates included the basic information like their gender, age and disease integrity, western medicine related indicators like White blood cell count, blood uric acid, ESR and CRP, TCM four diagnostic data. All dates are collected for researching and exploring the distributed regularity of TCM symptoms, defining the syndrome differentiation and treatment of gouty arthritis, on the basis of the features of TCM symptoms of gouty arthritis evaluation and research, and providing the useful basis for the clinical treatment of gouty arthritis in order to better serve the clinical.MethodsIn this study, we used a cross-sectional survey research method. We designed a TCM symptom information collection table for this study according to the clinical characteristics and accompanying symptoms of gouty arthritis. Then we filled in the questionnaire with the collecting related inpatient’s information during 2012-2015, which in Guangdong Provincial Hospital of Chinese Medicine.We establish the information data,base on the requirement of the statistics.We also entry and check out the data. Finally, we summarize and classify the syndrome of gouty arthritis by the factor analysis and cluster analysis method of SPSS22.0 software.Results1.This study to analyze 203 patients, we can see the male to female ratio is about 4.49:1. Patients are between 23-84, the average age was 60.21 years old while the mainly concentrated in the onset age 51-80 years old. Patients are attacked in winter and spring, spring and summer season alternation and cold climate.2. Most patients with clinical symptoms of TCM are respectively are joint pain, redness and swelling, fever, dry mouth, thirst, and trapped burnout, frequency of urine, constipation, eat less. Their tongue and pulse condition with dark red tongue, tongue collateral circuity, yellow greasy moss, pulse string sliding,pulse or pulse string.3. According to the factor score, the TCM syndrome type consists of damp and hot accumulate knot, dampness and blood stasis, blood stasis heat block spleen deficient, liver and kidney Yin deficiency, damp and hot accumulate knot the frequency has been the highest, which accounted for 38.42%, which has been the main syndrome types for gouty arthritis. Proportion of each syndrome types are as follows:damp and hot accumulate knot＞dampness and blood stasis>blood stasis hot wet stasis block spleen deficien＞liver and kidney Yin deficiency.4. According to 203 patients,153 cases of patients has the X-ray examination, inspection results show that conforms to the change of 52 cases of gouty arthritis, accounting for 25.62%of the total. According to the blood uric acid in patients with average sort visible damp and hot accumulate knot the blood uric acid level is highest, the second respectively wet stasis syndrome, blood stasis heat block spleen deficient, liver and kidney Yin deficiency syndrome. Damp and hot accumulate knot has the high levels of white blood cells than other kind of this syndrome,liver and kidney Yin deficiency syndrome of white blood cells had the lowest levels. In different syndrome types, ESR and CRP also not in identical,stasis hot block in the average of ESR and CRP were the highest, in the liver and kidney Yin deficiency syndrome the of ESR and CRP were the lowest. By the single factor analysis of variance showed that there was obvious difference in different syndrome type of white blood cells(p＜0.05),the visible differences between different syndrome type of white blood cells level. Between different syndrome type blood uric acid, the ESR and CRP contact has no obvious difference (p>0.05).ConelusionGouty arthritis is common relatively in patients with symptoms of joint pain, swelling and fever, dry mouth, thirst, body trapped burnout, frequency of urine, constipation, eat less and so on. Their tongue pulse condition has dark red tongue, tongue collateral circuity,yellow greasy moss, pulse string, pulse or pulse string. The TCM syndrome type consists of damp and hot accumulate knot, dampness and blood stasis, blood stasis heat block spleen deficient, liver and kidney Yin deficiency. Damp and hot accumulate knot card has been more patients than others in this disease. By the single factor analysis of variance showed that there was obvious difference in different syndrome type of white blood cells, the visible differences between different syndrome type of white blood cells level. Between different syndrome type blood uric acid, the ESR and CRP contact has no obvious difference. Damp and hot accumulate knot is one of the main syndrome types for gouty arthritis. This study may preliminary made it clear for TCM syndrome characteristics, provide the basis for clinical syndrome differentiation and treatment of gouty arthritis, but its accuracy and scientific remains to be verified.
